combined synopsis/solicitation for a commercial buy prepared in
accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only synopsis and solicitation that will be issued.
Proposals are requested in response to this synopsis/solicitation. A
written solicitation will not be issued. Solicitation F29650-99-R0500
is issued as a request for proposals. This solicitation and the
incorporated clauses and provisions are those in effect through Federal
Acquisition Circular 97-11. The SIC code is 8299 and the small business
size standard is $5.0 Million. This solicitation is set aside for small
business. The description of services follows: PERIOD OF PERFORMANCE:
17 May 1999 through 30 Sept 1999. The contractor shall provide Mishap
Investigation (Non-Aviation) [MINA] course instruction to include all
qualified instructors, course material prepared in conjunction with
applicable Air Force Instructions, all course supplies, evaluation of
student performance, all tests and examinations, course completion
certificate for each student who satisfactorily completes each course,
prepare and administer a course critique program to train ground,
weapons, and space safety personnel in the techniques and procedures of
investigating all classes of mishaps, including high accident potential
mishaps. The contractor shall provide instruction in the following
areas: Administration, Orientation, and Graduation; Investigation
Techniques; Engineering and Material Factors in Investigation; and
Human Behavior and Culture. 6 Courses at $__________each for a total
cost of $____________. Option I _ Provide MINA Courses on-site at
Kirtland AFB as requested between 01 JUN 99 through 30 SEP 99. 2
Courses at $________ each for a total cost of $____________. Option II
_ Provide MINA Courses off-site as requested between 01 JUN 99 through
30 SEP 99. 2 Courses at $___________ each for a total cost of
$_____________. Option I and Option II may beexercised at any time
during the period of performance. Award will be made on the basis of
best value to the Government, considering price, company
qualifications, instructor(s) qualifications, proposed course
structure, and outline of presentations. Instructor(s) must meet
qualification requirements as follows: Instructor(s) shall be
professionally qualified, experienced, and effective in each assigned
area of instruction (i.e., Investigation Techniques, Engineering
Factors in Investigation, and Human Behavior and Culture);
Instructor(s) shall be able to present technical information related to
investigation techniques, engineering factors, and human behavior and
culture; Instructor(s) shall be able to present mishap investigation
techniques, to include initial procedures, collection and analysis of
evidence, witness interviewing, development of findings, causes, and
recommendations, releasing information to the public, and compliance
with applicable Air Force Policy Directive/Instruction reporting
requirements; it is desired, but not required that instructor(s) have
served on a USAF Class A mishap investigation board; Instructor(s) are
required to have completed a four-year degree from an accredited
educational institution; it is desired, but not required, that
instructor(s) have completed a Masters Degree from an accredited
educational institution; Instructor(s) shall be active in safety
program administration and safety course instruction for a minimum of
two years during the past three years; it is desired but not required,
that instructor(s) be affiliated with professional safety
organizations. Delivery is FOB destination. The DO rating is S10. All
offerors shall include with their offers a completed copy of provision
52.212-3, Offeror Representations and Certifications_Commercial Items
(available upon request). The following clauses apply to this
