This will be a non-personal service contract and medical liability insurance in the amount of $1.0 million will be required. This requirement will be accomplished utilizing Best Value Commercial Item simplified acquisition procedures. The contractor will work weekdays during hours established by the Clinic Supervisor, normally not to exceed 8 hours a day, 16 hours a week, approximately 13 patients/day for the Alameda, CA and Seattle, WA locations and 8 hours a day, 1 day per week, approximately 13-16 patients/day for the Petaluma, CA Kodiak, AK locations. Period of performance will be 01 October 1999 through 30 September 2000 with three option years. The Request for Proposals will be issued on or about 22 June 1999 and close on or about 30 July 1999.
