Margaret M. Cook 602-222-6405 Provide anesthesiologist services to the
Carl T. Hayden VA Medical Center, Phoenix, Arizona. Estimated at three
full time equivalent (FTE) assuming an eight-hour workday, five days
per week, although at times the workload may require additional time
for completion. Requirements shall include: administering all forms of
anesthesia, including but not limited to general, spinal, epidural and
one lung. This shall also include services to conduct supportive
therapy, pre-operative evaluation of patients and central-line
placement. Provide monthly CME for a minimum of one hour for MD's,
CRNA's, nurses and others as appropriate. Furnish technical and/or
professional direction of CRNA's anesthetists and support personnel
employed by the VA. Furnish consultations referable to Anesthesiology.
Furnish on-call services to the VA Medical Center. Provide technical
and professional direction of postoperative care/therapy of patients in
Post Anesthesia Care Unit and in the Surgical, Intensive Care Unit, in
conjunction with the Surgical Staff. Provide input into the
performance evaluations of the CRNA's. Provide technical and
professional direction for Quality Assurance and Improvement program in
anesthesia. Participate in administrative duties as directed by the
Chief, Surgical Service to include but not limited to VISTA and patient
record entries. This is a Non-Personal Services contract as defined in
Federal Acquisition Regulation (FAR) 37.101. The Contract employees
furnishing services under this contract are not subject to the
day-to-day supervision and control of employees of the United States.
Quality surveillance shall be provided by the COTR. The Contractor
shall have the sole responsibility and perogative of selection,
assignment, reassignment, transfer, supervision, management, and
control of Contract Anesthesiologists in performance of this SOW. VA is
not hiring individuals to perform under this contract and shall not
hold "selection interviews" of any kind. (WILL VERIFY QUALIFICATIONS).
However, the Contractor shall not employ any persons for work on this
contract who are not fully qualified. Proof of qualifications will be
requested in the form of resume, reference, credentials,
certifications, etc. Period of contract will be a base period of one
year with four (4) options to renew for four (4) additional one year
periods, not to exceed a total contract period of five (5) years.
Solicitations are being requested under Public Law 104-262 The Veterans
Health Care Eligibility Reform Act of 1996 and Title 38 United State
Code Section 8153, with a Commercial Items format of terms, conditions,
and provisions/clauses. Solicitation is issued under FAR Part 12, as a
Commercial Services requirement, for Unrestricted Competition, with no
Price Evaluation Adjustment for Small Disadvantaged Business Concerns
under FAR 52.219-23, since price will not be the primary factor of
selection. Solicitation documents will be available on or about May 10,
1999; offers are due no later than COB on June 9, 1999. Solicitation
