The medical facility shall be responsible for furnishing all necessary professional health care services, including but not limited to diagnosis, consultations, treatment, reporting, management oversight and training. The professional services required include Inpatient/Outpatient Hospital Services; Inpatient/Outpatient Physician Services and Outpatient Insitution Services. These services shall include but will not be limited to Ophthalmology, Orthopedics, Dermatology, Otorhinolarynogology, Nephrology, Gastroenterology, Pathology, Anesthesiology, Internal Medicine, Chemotherapy, Cardiovascular Surgery, Detoxification, Psychiatry, Cardiology, Urology, General Surgery, Radiology, Infectious Disease, Endocrinology, Geriatrics, Dialysis, Obstertrics and Geynecology, Neurology, Vascular Surgery, Pulmonary Medicine, Outpatient Physical Therapy, Radiation Therapy, Oral Surgery, Drug Abuse Therapy, Laboratory Services, Nuclear Medicine, Plastic and Reconstructive Surgery, Podiatry and Neurosurgery. Location of the medical facility shall be within miles of the Federal Correctional Institution, 5701 8th Street, Camp Parks, Dublin, CA 94568. The anticpated period of contract performance will be; Base Year; Date of Award through 12 months from award date; Option Period #1; 13 months from date of award through 24 months from date of award; Option Period #2; 25 months from date of award through 36 months from award date; Option Period #3; 37 months from date award through 48 months from award date; Option period #4; 49 months from date of award through 60 months from awarfd date.
