Contracting Officer 1. This is a combined synopsis/solicitation for
commercial services prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; Requests
For Quotations (RFQ) are being requested and a written solicitation
will not be issued. Submit offers on RFQ # 903. The solicitation
document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 97-11. This acquisition will be
processed under Simplified Acquisition Procedures and will include both
fixed price and indefinite delivery/indefinite quantity items. See
Numbered Note #1. The associated Standards Industrial Classification
Code (SIC) for this procurement is 8322, small business size is $5
million. This purchase order will be for a period of 12 months, with
four (4) one-year options. SCHEDULE: The National Institute of Allergy
and Infectious Diseases, Rocky Mountain Laboratories (RML), Hamilton,
Montana has a requirement for an Employee Assistance Program (EAP).
Contractor shall furnish all labor, management, supervision, materials
and documentation to provide an EAP to employees and their immediate
family members who suffer from work-related, marital, family,
emotional, mental, financial, legal, alcohol and drug problems. The
Contractor shall provide personnel who is/are trained and certified
professional(s). A Professional is defined as a licensed or certified
psychologist, social worker, employee assistance professional or
addictions counselor. Professional Counselor(s) shall be within a
65-mile radius of RML: Firm Fixed Price Work: Supervisor Training
Services (1 session annually); Employee Orientation (1 annual); EAP
Display Posters (20 ea.); EAP Program Brochures (200 ea.); Survey
Reports (1 per case). Indefinite Delivery/Indefinite Quantity Work:
Employee Assistance Services (Not to exceed 100 hr./yr.); Critical
Incident Stress Debriefing (Not to exceed 16 hr./yr.); Supervisory
Consultation Services(Not to exceed 20 hr./yr.); Facilitation in the
Workplace (Not to exceed 40 hr./yr.) Services to commence not more than
30 days after receipt of order (ARO). Delivery location is Rocky
Mountain Laboratories, 903 S. 4th Street, Hamilton, MT. The following
