Q -- MEDICAL SERVICES: PRIMARY CARE SERVICES SOL RFQ 69D-276-99 DUE 062599 POC Bruce Blackburn, (414) 902-5402 Provide primary/mental health care services on an outpatient basis for approximately 900 veteran beneficiaries living in the Upper Peninsula of Michigan and north central Wisconsin(SIC is 8062 -- General Medical and Surgical Hospitals). An estimated usage of four (4) visits/patient/year are to be provided at a location that is within ten miles of the city center of Ironwood, MI. At a minimum these services shall include: 1) Primary care services (i.e. medical history, physical exam, assessment of health status, routine labs, case documentation, and outpatient management of a variety of medical conditions). 2)Disease prevention and treatment related to women's health/illness. 3) Compliance with VA customer service standards. This contract will be for a period of one year with two additional years at the option of the VA. It is anticipated that services under this contract should begin approximately mid-August 1999.
