VA Upstate NY Healthcare System, Network 2, has a requirement to
purchase two each phacoemulsifier units and two posterior vitrectomy
units, along with maintenance of equipment, and associated ophthalmic
products. This is a combined synopsis/solicitation for commercial items
prepared in accordance with the format in Subparts 12.6 and 13.5 of the
Federal Acquisition Regulation, as supplemented with additional
information included in this notice. The following incorporated FAR
clauses and provisions are those in effect through FAC 97-11 and are
applicable to this solicitation: 52.212-1 Instructions to Offerors --
Commercial Items; 52.212-2 Evaluation -- Commercial Items; 52.212-3
Offeror Representations and Certifications -- Commercial Items;
52.212-4 Contract Terms and Conditions -- Commercial Items; 52.212-5
Contract Terms and Conditions required to Implement Statutes on
Executive Orders -- Commercial Items and Y2K Warranty clause. All
responsible sources who can meet the VAs' requirements and provide
items as listed may respond to this solicitation by submitting cost and
technical proposals. This announcement constitutes the only
solicitation and written solicitation will not be issued. Contractors'
quotes are hereby being requested and response to requirements listed
below shall be submitted on Contractors' letterhead or bid form and
must address all requirements listed and must contain the name, title
and signature of person authorized to submit proposals/bids on behalf
of the Contractor. Descriptive literature for each item offered must
also be included in proposal package. Representative samples or system
demonstrations may be requested by the Contracting Officer at her
discretion and will be due within 5 days of request. The Government
intends to award a Firm Fixed price contract. The Government requires
FOB destination. FOB points will be VA Medical Center, 113 Holland Ave,
Albany, NY 12208 and VA Medical Center, 3495 Bailey Avenue, Buffalo, NY
14215. The VA has a requirement for phacoemulsification and anterior
vitrectomy equipment that will provide state of the art technology to
perform cataract and retinal surgery in a shorter time frame, with
minimum risk to patients, and maximum confidence to surgeons and
training surgeons. The following characteristics are required in the
phaco unit: Aspiration by-pass phaco tip; small incision phaco tip with
.175 mm by-pass hole allowing for continuous flow; continuous flow of
fluid to contribute to dissipating thermal build-up; light weight ultra
sonic handpiece driven by it's own multi-freq. ultrasonic generator and
supported by it's own microprocessor; system capability that interfaces
with fluid vented peristaltic pump; angled phaco tip; peristaltic pump
allowing for independent control of vacuum and aspiration levels;
constant admittance tuning that provides tuning of ultrasonics via
real-time sampling 100-time/second monitoring stroke length and
frequency; traditional phaco, pulse phaco, bimodal and burst phaco;
cordless, infra-red remotecontrol, capable of operating all primary
functions from the sterile field; guillotine cutting vitrectomy probe
with adj. port opening; memory storage holding up to four memory
settings for up to 15 physicians; continuous irrigation mode regardless
of physican foot-pedal position; and small bore aspiration tubing. The
vitrectomy units shall be evaluated on their interfacing capabilities
and these parameters -- vacuum, cutting, pak configuration,
illumination, fragmentation, air pump, scissors, diathermy, silicone
injection, silicon extraction, footswitch, memory, reflux, retinal
tamponade, and anterior capability. Technical capabilities of the
contractor, maintenance programs offered, as well as pricing for
equipment, maintenance, and pricing of all associated products
(disposables, intraocular lenses,etc.) will be considered to determine
"best value" to the Government. Technical proposals must furnish
detailed data and information concerning the purchase of equipment, as
well as technical capability and past performance history of the
contractor (including three references with names and phone numbers.)
Pricing proposals must include equipment purchase pricing, maintenance
programs offered with details and pricing, and a listing of all
associated products which may be required for surgical procedures, to
include item numbers, descriptions, and pricing. Pricing shall be
submitted for a base year, plus four option years for the purchase of
maintenance and associated products. Evaluation and award of a contract
may be made with or without discussion with the offeror(s). Submit all
