is a combined synopsis/solicitation for commercial items prepared in
accordance with the format in Subpart 12.6, as supplemented, with
additional information included in this notice. This announcement
constitutes the only solicitation; bids are being requested and a
written solicitation will not be issued. The solicitation document and
incorporated provisions and clauses are those in effect through
Federal Acquisition Circular (FAC) 97-11. The solicitation is issued as
Request for Proposal F22600-99-R0084 and is 100% set-aside for small
businesses. SIC -7359. Size Standard -- $5 million. This is a DO rated
order with a priority identification of B1. The requirement is for all
labor, parts, materials, transportation, turn-key project management
and other items and services necessary to provide, transport, install
and lease a standard 48x60' pre-engineered building (new or
refurbished), to include a drive-up window addition (factory or stick
built), to be equipped and configured for use as a temporary satellite
pharmacy facility at Keesler Air Force Base, Mississippi, in
accordance with Statement of Work MAHG 98-8011 and the Drawings. Bids
must be submitted for each of the following Line Items: 0001 Cost (Lump
sum) associated with construc-tion, delivery, set-up, skirting and any
other work required for occupancy. Set-up must be completed no later

Offerors-Commercial Items (Aug 98), incorporated by reference, applies
to this acquisition and is amended to read: Submit signed and dated
offer to 81 CONS/LGCV, 310 M street, Room 102, Keesler AFB MS
39534-2147 at or before 1530 (3:30 p.m.) CDT, 7 Jun 99. Offers must be
signed, dated and submitted on company letterhead or bid form and may
be submitted via fax to (228) 377-1811, attn: Tom Kocurek. Each
offeror shall include a completed set of the provision at FAR 52.212-3,
Offeror Representations and Certifications-Commercial Items (May 99).
5253.232-9000 REMITTANCE ADDRESS (MAY 1996) If the remittance address

monthly on the submission of proper invoices for the services
performed during the preceding month. Invoices shall be submitted on a
monthly basis covering the services performed during the preceding
month. The contractor's monthly invoice shall be submitted to the
address in block 18a on the SF 1449. FAR clause 52.212-4, Contract
Terms and Conditions -- Commercial Items (May 99), is incorporated by
reference and applies with the following Addendum: (c) Changes. Changes
in the terms and conditions of this contract may be made only by
written agreement of the parties with the exception of administrative
changes, such as changes in the paying office, appropriations data,
etc., which may be changed unilaterally by the Government. FAR
52.212-5, Contract Terms and Conditions Required to Implement Statutes
or Executive Orders -- Commercial Items (May 99) applies to this
solicitation. The following items of subparagraph (b) apply: 5, 7, 12,
13, 14, 15, & 22. DFARS 252.212-7001, Contract Terms and Conditions
Required to Implement Statutes or Executive Orders Applicable to
Defense Acquisition of Commercial Items (Oct 98), and in subparagraph
(b), 252.225-7012, apply. The following additional clauses are
incorporated by reference: 52.217-8 OPTION TO EXTEND SERVICES (AUG
1989) For the purpose of this clause, the Contracting Officer may
exercise the option by written notice to the Contractor within 15
calendar days prior to the expiration date of the contract. 52.217-9
OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 1989) For the purpose of
this clause the blank(s) are to be completed as follows: (a) within 15
calendar days prior to the expiration date of the contract.; (c) not
to exceed 31 months. (The 60-day preliminary written notice stipulated
in the above clause will not apply to the first option period since
the Government anticipates exercise of the first option, subject to

52.232-18 AVAILABILITY OF FUNDS (APR 1984)(Applicable for option
periods) 52.237-2 PROTECTION OF GOVERNMENT BUILDINGS, EQUIPMENT, AND
VEGETATION (APR 1984) 5352.217-9000 OPTION CLAUSE LIMITATION NOTICE
(JUL 1994) This contract contains two option provisions, (i) Option to
Extend Services, and (ii) Option to Extend the Term of the Contract
(see FAR 52.217-8 and FAR 52.217-9). Either or both may be exercised
unilaterally by the government. The clause entitles "Option to Extend
the Term of the Contract" will not be exercised after any exercise of
the "Option to Extend Services" clause 5352.223-9001 HEALTH AND SAFETY
ON GOVERNMENT INSTALLATIONS (JUN 1997) (a) In performing work under
this contract on a Government installation, the contractor shall: (1)
Comply with the specific health and safety requirements established by
this contract; (2) Comply with the health and safety rules of the
Government installation that concern related activities not directly
addressed in this contract; (3) Take all reasonable steps and
precautions to prevent accidents and preserve the health and safety of
contractor and Government personnel performing or coming in contact
with the performance of this contract; and (4) Take such additional
immediate precautions as the contracting officer may reasonably require
for health and safety purposes. (b) The contracting officer may, by
written order, direct Air Force Occupational Safety and Health
Standards (AFOSH) and/or health/safety standards as may be required in
the performance of this contract and any adjustments resulting from
such direction will be in accordance with the Changes clause of this
contract. (c) Any violation of these health and safety rules and
requirements, unless promptly corrected as directed by the contracting
officer, shall be grounds for termination in accordance with the
Default clause of this contract. 52.212-2 EVALUATION -- COMMERCIAL
ITEMS (OCT 1997)(a) The Government will award a contract resulting from
this solicitation to a single responsible offeror who submits the
lowest price proposal. (b) Options. The Government will evaluate offers
for award purposes by adding the total price for all options to the
total price for the basic requirement. The Government may determine
that an offer is unacceptable if the option prices are significantly
unbalanced. Evaluation of options shall not obligate the Government to
exercise the option(s). (c) A written notice of award or acceptance of
an offer, mailed or otherwise to the successful offeror within the time
for acceptance specified in the offer, shall result in a binding
contract without further action by either party. Before the offer's
specified expiration time, the Government may accept an offer (or part
of an offer), whether or not there are negotiations after its receipt,
unless a written notice of withdrawal is received before award.
52.216-1 TYPE OF CONTRACT (APR 1984) The Government contemplates award
of a Firm Fixed Price contract using Simplified Acquisition Procedures
resulting from this solicitation. 52-233-2 SERVICE OF PROTEST (AUG
1996) (a) Protests, as defined in section 33.101 of the Federal
Acquisition Regulation, that are filed directly with an agency, and
copies of any protests that are filed with the General Accounting
Office (GAO), shall be served on the Contracting Officer (addressed as
follows) by obtaining written and dated acknowledgment of receipt from
81 Contracting Squadron/LGCV, 310 M Street, Suite 102, Keesler AFB MS
39534-2147. (b) The copy of any protest shall be received in the office
designated above within one day of filing a protest with the GAO.
52.237-1 SITE VISIT (APR 1984) Offerors or quoters are urged and
expected to inspect the site where services are to be performed and to
satisfy themselves regarding all general and local conditions that may
affect the cost of contract performance, to the extent that the
information is reasonably obtainable. In no event shall failure to
inspect the site constitute grounds for a claim after contract award.
