Pomraning This is a combined synopsis/solicitation for commercial items for the Department of Veterans Affairs and is prepared in accordance with the format in Subpart 12.6, as supplemented with additional infomation provided in this notice. This annoucement constitutes the only solicitation; proposals are requested; and a written solicitation will NOT be issued. This solicitation is issued as a Request for Quotes: RFQ 512(B)-97-99. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-11. NOTE #1 applies. The Standard Industrial Classification is 8071. Small business size standard is $5.0M. The VA Maryland Health Care System (VAMHCS) requires the services of a contractor to assist in performing autopsies. The following requirements must be met: Contractor must be available seven days a week form 8 a.m. to 2:30 p.m., Eastern Standard Time. The VA will notify the contractor when an autopsy must be performed. Based on past performance statistics, it is estimated that six autopsies will be performed each month. This is an estimate only. The VA is not obligated to any specified amount of products and/or services, but shall be obligated to provide payment for all items/services requested in accordance with this contract. The contractor is responsible for assisting the pathologist and resident in performing the autopsy. The contractor is also required to clean all instruments, tables, sinks, and any other equipment used, in accordance with the standard operating procedures for autopsies and must strictly follow the safety procedures as outlined by written policies. A clean and safe environment must be maintained at alll times. The contractor must provide personnel to assist the anatomic pathology section which requires an estimate of three days per week, four to six hours per day. Contractor must assist in putting away supplies, locate tissue and reports, and when necesssary, assemble slides for teaching purposes for the pathologist. Contractor must report to the histopathology supervisor upon arriving for sign-in purposes and assignments. The following incorporated clauses and provisions are those in effect through FAC 97-11 and are applicable to this acquisition: 52.212-1, Instructions to Offerors -- Commercial Items, includes by addenda: 52.233-2; 52.212-2, Evaluation Commercial Items, apply to this acquisition with evaluation criteria to include: (1) Past Performance, (2) Technical Capability, and (3) Cost. These factors are listed in descending order of importance. Contractor is to submit three references which includes name and phone number of contact person. Offerors are to include a completed copy of the FAR provision 52.212-3 (including VAAR 852.219-70), with their offer. Original proposals must be submitted in writing on company letterhead to: Carol L. Pomraning, Contracting Officer (90C), VAMHCS, 10 North Greene Street, Baltimore, MD 21201. No fax or telephonic proposals will be accepted. The Government intends to evaluate proposals and award the contract without discussion with offerors. Therefore, the proposals should include the offeror's best terms. Award will not necessarily be made to the offeror with the lowest offer.
