(CDC) intends to solicit a quotation for the development of a refined
framework (surveillance program logic model) for state-based traumatic
brain injury (TBI) surveillance, and assess the usefulness of the data
collected by defining who uses the data, the purposes for which the
data are used, the data elements used, and other data elements of
interest not currently collected. The focus of this evaluation will
include the development of case studies of individual state programs,
creating a model of critical indicators (process and outcome) that can
be used to assess the efficiency and effectiveness of these programs
at all stages of development and implementation, and describing actual
and potential uses for the data. This is a 100% small business
set-aside under SIC code 8733 with a size standard of $5 million
average annual receipts for the three fiscal years. Simplified
Acquisition Procedures will be utilized and written quotations will be
solicited on or about 6/15/99. All interested parties are requested to
