this Request for Proposal is DABT59-99-R-0011. This is a combined
synopsis/solicitation for commericial items prepared in accordance with
FAR 12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; a written
solicitation will not be issued. The Standard Industrial Code 0783
applies and limits this solicitation to offerors with less than $5
million in annual receipts. PROPOSALS ARE REQUESTED FOR TREE TRIMMING
AND BRUSH REMOVAL SERVICES FOR THE FOLLOWING THREE LINE ITEMS (in
Dollar Amount per Hour): (0001) TWO-MAN CREW, 60' BUCKET TRUCK WITH 15
CUBIC YARD CHIPPER DUMP BODY WITH A MINIMUM 18" CHIPPER, ALL CHAIN
SAWS AND HAND TOOLS AS NECESSARY. ESTIMATED QUANTITY: 952 TOTAL HOURS
X UNIT PRICE PER HOUR = TOTAL (0002) TWO-MAN CREW, 80' BUCKET TRUCK AND
ALL CHAIN SAWS AND HAND TOOLS AS NEEDED. ESTIMATED QUANTITY: 952 TOTAL
HOURS X UNIT PRICE PER HOUR = TOTAL (0003) TWO-MAN CREW, 15 CUBIC YARD
CHIPPER DUMP BODY, 18" CHIPPER, AND ALL CHAIN SAWS AND HAND TOOLS
NEEDED. EST QTY: 952 HR X UNIT PRICE PER HR = TOTAL The contractor
shall provide all labor, materials, equipment, and supervision to chip,
cut, and remove dead trees and fallen/damaged limbs as directed by the
government throughout FT Lee (EXCEPT the family housing areas). Chips
and logs may be dumped at a designated area on FT Lee. It is
anticipated that 50% of the work will be performed in the Fall or
leafless state of the trees when the damage can be more readily
identified. Hours of operation are 8:00AM -- 4:30PM, Monday-Friday; and
weekends/holidays if approved in writing by the Contracting Officer.
The Government contemplates award of a Firm-Fixed Priced Requirements
contract. Orders will be placed on a daily, weekly, or monthly dasis.
The contractor will only be paid for actual hours used. Quantity of
hours is estimated and not guaranteed. The contractor will have 48
hours to start work after notification by phone and will notify the
Qualilty Assistant Evaluator (QAE) by phone every morning when arriving
on the FT Lee post. The QAE will b eappointed upon contract award. The
contractor shall abide by all OSHA laws and ensure safety practices
are enforced to protect government property and post personnel. The
12-month contract period is tentatively scheduled for JUL 1999-JUN
2000. Addendum 52.216-18: Orders may be issued from the date of award
thru 12 months thereafter. Addendum 52.216-21: Contractor shall not
make any deliveries after 12 months from the date of contract award.
INSTRUCTIONS TO OFFERORS, FAR 52.212-1 Addendum-Commercial: Past
Performance: Provide last five government or commercial contract
references for same or similar type services within the last 3 years to
include contract number, contracting officer's name & phone number.
Provide key personnel to be used on this contract. Technical: Provide
list of Equipment to include Make, Model & Year. Offerors are to
include the information requested of the provisions at FAR 52.212-3,
OFFEROR REPRESENTATIONS & CERTIFICATIONS-Commercial items with the
offer to include Federal Tax ID, DUNS Number, and Cage Code. Also
reference the solicitation number DABT59-99-R-0011 on the proposal.
EVALUATION, FAR 52.212-2, Addendum-Commercial: Evaluation Factors (1)
Technical Capability (2) Past Performance and (3) Price. Applicable
Clauses include FAR 52.212-4, Contract Terms and Conditions-Commercial;
FAR 52.212-5 Terms and Conditions required to Implement Statutes or
Executive Orders-Commerical with the following FAR clauses in paragraph
(b) applicable: Equal Opportunity 52.222-26, 52.222-35, 52.222-36,
52.225-3; and Mandatory Central Contractor Registration (CCR) DFAR

issued from date of award thru 12 months thereafter. FAR 52.216-21
Addendum: Contractor shall not make any deliveries under this contract

Addendum:(a)(1) MINMUM ORDER: 1 HOUR PER LINE ITEM (b)(1) MAXIMUM
ORDER: 100 HOURS PER LINE ITEM (b)(2) MAXIMUM COMBINATION PER ORDER:
300 HOUR TOTAL ORDER. (b)(3) WITHIN 7 Calander period. SEE Note 1. The
government may award this contract on the basis of initial offers
received without discussions. Therefore, the offer should contain the
best terms from a price and technical standpoint. This award may be
