Q -- PRIMARY CARE CLINIC SOL RFP-597-32-99 DUE 062199 POC Bonnie Obermier, 402-486-7880 The VA Greater Nebraska Health Care System has a requirement to utilize a pre-existing clinic in the North Platte, Nebraska, area complete with physicians and personnel required for primary care and routine primary care services. At the time of response to this solicitation, clinic must already be JCAHO accredited and must have either on-site lab, x-ray, and Pharmaceutical capabiity or a least be able to demonstrate that a contractural relationship exists to provide off-site lab, x-ray, and pharmaceutical services to the existing clinic. Clinic must be located within a thirty (30) mile radius of North Platte, Nebraska and must be located within VA Greater Nebraska Health Care System's primary service area. Physicians, Nursing staff, technicians operating under the resulting contract must meet all evaluations criteria and other professional certifications, and licenseing required to operate in such a clinic. The qualifications of such personnel shall also be subject to review by the Chief Of Staff of this medical center and approved by the Greater Nebraska Health Care System Director. There are approximately 1200 patients in the service area and the VA Greater Nebraska Health Care System intends to negotiate a capitated rate per patient.
