MODIFICATION TO BAA 99-22: The following information is provided as a clarification to the use of Standard Form 1411 and the corresponding reference to FAR provision for cost proposal preparation. Under paragraph, "COST PROPOSAL," the cost proposal shall be prepared in general accordance with Table 15-2 of FAR 15.408, and shall include a Contract Pricing Proposal Cover Sheet, similar to Standard Form 1411, with all supporting data in order to allow for a complete review by the Government. The use of Standard Form 1411 is acceptable to the Government. In addition, prime contractors are responsible for assuring that all proposed subcontracts in excess of $500,000 are supported by an executed Standard Form 1411 or Contract Pricing Proposal Cover Sheet similar to Standard Form 1411.
