or (810) 307-4389 SCHEDULE: The Contractor shall submit a complete work
schedule to the COR office in building #970, room 223 prior to the
start of construction for approval. The schedule shall indicate
delivery dates for material, start of construction, activity
description with start date and completion date and pertinent remarks
effecting the schedule. DAILY REPORT: The Contractor shall submit a
"Daily Report" form to the COR office each morning work is scheduled.
The submittal shall be turned in by 0800 for the work to be performed
that day. Failure to submit this form as required shall be
justification for the approval authorities to disapprove the Contract's
work for that day. The "Daily Report" shall be given to the Contractor
at the pre-construction meeting. EROSION, SEDIMENT AND DUST CONTROL:
The Contractor shall be responsible for erosion, sediment and dust
control. Drainage structures within the direct confluence of
construction shall receive sediment barriers. Haul roads shall be
continually cleaned and dustcontrolled throughout the workday as the
need arises. Failure to perform this work will justify a stoppage of
work until corrections are made. WATER CONNECTIONS: No water
connections shall be made until the new water main has passed both the
bacteriological and hydrostatic leakage tests. The water main shall be
capped at connection locations, chlorinated and pressure tested and
then connected only after the testing has been approved and passed. The
Contractor shall give 48 hour notification to all users affected by any
utility shut down. All repairs or connections shall be made
expeditiously. Material and equipment and labor shall be in place prior
to shutting off the utility. DISTURBED AREAS, DAMAGED PROPERTY AND
LANDSCAPING: All disturbed areas or damaged property shall be returned
to their original or better condition at the Contractor's expense.
Disturbed areas requiring landscaping shall receive the following work:
fill with select material, compact, rough grade, fine grade, 2" topsoil
placement, seeding and mulch. Topsoil shall be screened. Seeding shall
consist of perennial rye (50%), creeping red fescue (50%) placed at a
rate of 150#/acre. Mulch shall be hay placed at a rate of 2 tons/acre.
AS BUILT DRAWINGS: The Contractor shall as built the construction
drawings after project completion. The as built shall show as built
locations of all installed water main, fittings and connections. The
person making the drawings shall note their name, company and date. Two
black line copies of the as built shall be given to the COR.
SUBMITTALS: The Contractor shall submit 5 copies of all work related
items to the COR located in Bldg. 970, room #223 prior to any
construction activity. The "submittal form", FIGURE 3 shall be filled
out and used for transmitting the submittals. Submittals shall include
all items of construction such as: water main pipe, fittings,
specials, valves, backfill, well, boxes, fire hydrant,corporations,
thrust block mix, aggregate analysis, detailed method of work
performance, schedule, descriptive literature including but not limited
to catalog cuts, diagrams, operation charts, test reports and
warranties. The approval of the submittals by the Government shall not
be construed as a complete check, but shall indicate only the general
method of construction and detail is satisfactory. Acceptance does not
authorize changes in the contract unless the Contracting Officer in
writing approves the change. The Contractor shall familiarize himself
with the location of the proposed work and the conditions under which
it must be performed, and shall furnish all necessary plant, labor,
material, and equipment to accomplish the Water Main. The work to be
performed at the U.S. Army Garrison -- Selfridge within the Michigan
