Pettis Memorial VA Medical Center, Loma Linda California, has a requirement for MOHS Micrographic Surgery (MMS) microscopically controlled removal of a skin tumor services for patients of the VA Medical Center at Contractor's facility. This will be a firm fixed price requirement contract for MOHS procedures, including local anesthesia, surgical excision, pathology readings, postoperative care, follow-up services, STAT request, supplies, equipment, furnish all labor management, supervisions and resources necessary to provide for contracted services. The provider's facility must be accessible and convenient for both wheelchair and ambulance patients and located within a reasonable proximity to the VA which is located at 11201 Benton Street, Loma Linda, California 92357. Rates will be per CPT-4 codes; approximately 60 procedures are required per year. Physicians who provide MOHS procedures must be specially trained dermatologist for the treatment of malignant neoplasms of the skin and mucosa and must be certified by an American Board of Dermatology. The performance period shall be for a base year from the effective date of award with two one-year renewal options. The VA intends to award a single contract and reserves the right to make multiple awards if in the Government's best interest.
