745-8465 This is a combined synopsis/solicitation for commercial items
prepared in accordance with the format in FAR, Subpart 12.6 as
supplemented with additional information included in this notice. This
notice constitutes the only solicitation; quotes are being requested
and a written solicitation will not be issued. This solicitation is
being issued as Request for Quotation number RFP 688-25-99. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular number 97-10. The SIC
code for this solicitation is 7338 with total revenues not to exceed $5
million. The small business size standard is 500 employees. This
solicitation is set aside for small businesses. Contractor shall
furnish on-site radiology transcription services at the Department of
Veterans Affairs Medical Center, located at 50 Irving Street,
Washington, DC 20422 in accordance with all terms and conditions of
this solicitation The contractor will be responsible for transcribing
imagingreports on-site into the VAMC Decentralized Hospital Computer
System (DHCP) using a Lanier Dictation System and the Radiology
Transcription Computer System within DHCP. Contract period is for six
months from date of award with a six-month optional extension, subject
to the availability of fiscal year funds. Contractor is required to
have specialized experience and be regularly established in the
business of Medical/Radiology transcribing services. The contractor
shall perform all work pursuant hereto in accordance with the
requirements and standards at the direction of the Imaging Service
Administrator Officer, acting under the delegated authority of the
Contracting Officer. The contractor shall provide transcriptionists
with a minimum of two or more years of recent experience transcribing
General Radiology and its sub-specialties: Ultrasound, Nuclear
Medicine, Computerized Tomography, Magnetic Resonance Imaging and
Vascular/Interventional Imaging. The material may be generated by as
many as ten (10) authors, many of which have English as a second
language, requiring the transcriptionists to adapt to variations in
accents, speech patterns, tone, voice volume, notes and style of
delivery, pronunciations and enunciation. Transcriptions are expected
to produce a minimum of 1000 lines -- a line is defined as eight inches
-- per eight (8) hour tour with a 98% transcription accuracy (per 100
words transcribed).Transcription services will be required Sunday
through Saturday as scheduled:Transcription support from 9:00AM --
5:30PM Eight (8) hour tour -- Monday through Friday with hour for
lunch; Transcription support from 10:00PM -- 6:30PM Eight (8) hour tour
-- Monday through Friday with hour for lunch, Transcription support
from 12:00PM -- 8:30PM Eight (8) hour tour -- Monday through Friday
with hour for lunch; Transcription support from 2:00PM until 6:00PM
Four (4) hour tour -- Saturday, Sunday and Holidays. Turn around time
for reports dictated between the hours of 8:00AM and 6:00PM
Mondaythrough Friday are to be transcribed and entered into the DHCP
computer system within two (2) hours. Reports dictated on weekends and
holidays are to be entered into the DHCP computer system by the end of
tour completion. Transcription services will be required for holidays.
National Holidays are described as the ten holidays observed by the
Federal Government. They are: New Year's Day, Martin Luther Kings'
Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor
Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and
any other day specifically declared by the President of the United
States to be a national holiday. Transcription will be typed with
single line spacing between paragraphs. Exact execution of Veteran
Administration (VA) format is mandatory. The transcriptionists are
responsible for correct spelling, including unfamiliar words, checking
of medical dictionaries and reference sources when needed for
distinguishing between similar sounding medical terms. All material
isto be transcribed in the tense dictated. The transcriptionists are
required to exercise judgment in discerning errors and omissions, which
are administrative in or clerical in nature. Those errors, which are
technical in nature, should be referred back to the dictator for
clarification or correction. No abbreviations are to be used when
typing final diagnosis and the nature of the operation and/or
procedure. Abbreviations in the text are to be limited only to those
commonly used in the laboratory values. The Contractor shall replace
any transcriptionists found deficient in his/her performance as
determined by the VAMC Imaging Service. Transcriptionists are required
to initial off on all reports. Contractor shall perform all work
pursuant hereto in compliance with all applicable Federal, state and
municipal statues and in compliance with all applicable by-laws of the
VAMC. The Contracting Officer in writing must authorize any additional
work outside the scope of this contract. Failure to get that approval
may result in the contractor not being compensated for that work. The
contractor shall be responsible for protecting the personnel furnishing
services under this contract. To carry out this responsibility, the
contractor shall provide the following for these personnel: workers
compensation, professional liability insurance, health examination,
income tax withholding, and-social security payments.The parties agree
that such personnel shall not be considered VA employees for any
purpose and shall be considered employee of the contractor. Contractor
guarantees total confidentiality of all VAMC records and information
contained therein, and all other related materials. Firms to be
considered must be within a 20-mile radius of this medical center. The
terms of this contract provide for all or none, which means that the
firm must be able to provide personnel for each tour of duty and these
individuals must perform the entire contract period. Failure of the
individuals to perform is cause for immediate default and termination
of the contract. The following provisions apply to this solicitation:

acceptance of offer, mailed or otherwise furnished to the successful
bidder within the time for acceptance specified in the offer shall
result in a binding contract without further actions by either party.
Offerors responding to this announcement shall submit an hourly price
for each category along with the information required in 52.212-1 and
a completed copy of provision 52.212-3 to VA Medical Center (688/90c),
Room 1B112, 50 Irving Street NW, Washington, DC 20422 by June 30, 1999
