Broider-301-443-5231 The Health Resources and Services Administration
intends to negotiate on a SOLE SOURCE basis with Summit Health
Institute for Research and Education, Inc. 1424 K Street, N.W.,
Washington, DC 20005. This procurement shall be processed in accordance
with the Simplified Acquisition Procedures in Part 13 of the Federal
Acquisition Regulations. The purpose of this procurement is to
establish a framework for a community-based diabetes demonstration
project which will broaden community participation in fostering health
promotion activities and health care policies that benefit people with
diabetes. The contractor shall: prepare a project workplan describing
the framework for a community-based diabetes demonstration project;
develop a criteria for selecting 5 pilot sites for participation in the
project; conduct the 5 pilot site visits; and prepare a final report.
The report shall include strategies for connecting multi-cultural
faith-based organizations to the health providers attached to the
Bureau of Primary Health Care funded Community Health Centers who have
participated in the special "diabetes training" sponsored by the
Institute for Health Care Management; profiles on each of the 5
demonstration sites; goals and objectives of the demonstration
projects; an explanation of their relationship to the Diabetes
Coalition Project (DCP); strategies for post project funding; and
strategies for replicating and funding additional community partnership
projects. The contractor shall also implement a strategy for forming a
working partnership with the Bureau's DCP staff and the National
Cultural Competency Center. The Summit Health Institute for Research
and Education, Inc. is deemed the only responsible source capable of
providing the required work and services. The period of performance is
one year. A Justification for Other than Full and Open Competition is
on file. The award shall be made using simplified acquisition
