synopsis/solicitation for commercial items prepared in accordance with
the format in FAR subpart 12.6 as supplemented with additional
information provided in this notice. This is a sole source acquisition
to Survival Incorporated of Seattle, WA. Supply of soft composite Body
Armor. This announcement constitutes the only solicitation; a quote is
being requested and a written solicitation will not be issued.
Solicitation number F65501-99-T-0239 is issued as a Request for
Quotation (RFQ). The incorporated document provisions and clauses are
those in effect through Federal Acquisition Circular 97-11. The
Standard Industrial Classification Code is 3843, small business
standard is 500 employees. Line Item 0001: 204 ea Soft composite body
armor ( balistic Panel Jacket) Manufacturer: Survival Inc part number:
BA-2/3A-R. This body armor unit must intergrate with the Air Ace
Survival Vest and the Air Arrest Rescue and Recovery Harness, both
manufactured by Survival Inc. Survival Inc is currently the only
authorized source of supply. Delivery terms shall be FOB destination
with delivery to Base Supply Receiving 5256 Gibson Ave, West End,
Elmendorf AFB, AK, 99506. Delivery shall be made No Later Than 090
calendar days after receipt of order. The following provisions and
clauses are incorporated and shall remain in full force in any
resultant purchase order; FAR Part 52.212-1; Instructions to Offerors,
Commercial Items is incorporated by reference. The provision at
52.212-2; Evaluation-Commercial Items, applies to the solicitation.
Award will be made based on past performance, delivery schedule and
price being evaluated as substantially equal. The government reserves
the right to make award on the initial quote received without
discussions. Each offeror shall include a completed copy of the
provision at FAR 52.212-3; Offeror Representations and Certifications
-- Commercial Items with its quote. FAR Part 52.212-4; Contract Terms
and Conditions-Commercial Items, is incorporated by reference and
applies to this acquisition. Note the following addenda to FAR
52.212-4: 1) Incorporate DFARS 252.204-7004; Required Central
Contractor Registration. 2) Reference Paragraph (1), replace FAR
52.232-33; Mandatory Information for Electronic Funds Transfer Payment,
with DFARS 252.232-7009; Payment by Electronic Funds Transfer (CCR).
Incorporate the clause by reference at 52.212-5; Contract Terms and
Conditions Required to Implement Statutes or Executive Orders --
Commercial Items. The Contractor shall extend to the Government their
standard warranty offered in similar types of sales. Acceptance of the
warranty does not waive the Government_s right with regard to the
other terms and conditions of the contract. In the event of a conflict,
the terms and conditions of the contract shall take precedence over the
warranty. The warranty period shall begin upon final acceptance of the
items listed in the schedule. This is a DO rated order with a priority
identification symbol of C9E. See CBD Note 22. A firm-fixed price
purchase order will be issued under the Simplified Acquisition
Procedures. Submit signed and dated offer to 3 CONS/LGCS, 10480 22nd
St., Elmendorf AFB, AK 99506 at or before 1530 (3:30 p.m.) Alaska Time
23 June 1999. Quotations shall be signed, dated and submitted on your
Company_s letterhead or quotation forms. Quotations may be submitted
via fax. All questions concerning this RFQ must be submitted in writing
and faxed to Ssgt Rick Crivens, (907) 552-2965. No telephonic responses
will be processed. Armor. Soft composite body armor (ballistic panel
jacket) must match air ace and "SRU-21/p" survival vests and rescue &
recovery harness (mfr. by Survival Inc). Shall be one-size-fits-all
construction, with shoulder/side protection; 4-point 4-point adj.
system w/quick release for rapid removal; silent quick-release
Waist/shoulder buckles to adjust fit; & "snap track" attachments for
integration w/snap track pockets and other "air ace" Equip. There shall
be no sharp edges or seams. Seams shall be sewn to the inside and
corners rounded. Armor shall be constructed of hermitically sealed
multi-layered, unidirectional aramid fiber Composit ballistic material
sewn in panels, with an aerial density of 1.2 Lbs/sq ft or less. Shall
be inherently waterproof, inherently buoyant (floats without foam etc)
and resistant to water degredation, UV light degredation, rot, mildew,
or abrasion. Unit shall be light And flexable so as not to impede in
the water or negatively impact one's ability to swim and stay afloat.
Panel covers shall have sewn in pockets for addition of soft trauma
packs or armor plates. It shall be fire retardent to FAR 25.853 & NFPA
701. Armor shall be certified by U.S. Department of Justice National
Law Enforcement & Correction Center, to level IIIA protection level as
defined by NIJ fragmentation V50BL(P). Additionally it shall allow
increased protection to NIJ Level IV through the use of soft trauma
packs or cermanic and other armor plates placed in the pockets for
them. Body armor is to be used by aircrew and ground crew members, for
long periods of time, over and under flight equipment or uniforms and
approved for use, by HEADQUARTERS PACIFIC AIR FORCES LIFE SUPPORT.
