Q -- EMPLOYEE ASSISTANCE PROGRAM SOL RFP 646-49-99 DUE 072299 POC Lynne Dickerhoff, Contract Specialist, 412-365-5441 The VA Pittsburgh Healthcare System is seeking qualified healthcare providers capable of performing services for the "employee assistance program" in the Pittsburgh, PA geographic area. It is contemplated that a single award will be made to a healthcare facility qualified to provide employee assistance such as, but not limited to assessment, counseling and referral services to employees of the VA Pittsburgh Healthcare System and their family members for the purpose of determining the early recognition, intervention, and resolution of work-related and personal problems in accordance with contract terms and conditions from October 1, 1999 through September 30, 2000, with two (2) one-year renewal options. Offeror must have at least three (3) sites in Allegheny County. The Government estimates the workload to be 2400 employees with each employee being seen an average of three (3) one hour sessions. The offeror will quote a capitation rate for the base year and each option year. The estimated issue date forthis solicitation is 7/1/99 and the estimated due date for proposals is 7/22/99.
