REQUIREMENT to upgrade government owned Abestos Containing
Material/Lead Based Paint (ACM/LBP) database from Microsoft Access 2.0
to theversion in use on the 3rd Civil Engineer_s server ( currently
Access 97, SR-2) see attached Statement of Work. Performance Period
contractor shall be completed within 10 months of start date.
Contractor shall start performance prior to 15 Sep 1999. This is a
combined synopsis/solicitation for Commercial Items prepared in
accordance with the format in subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; quotes are being requested and a
written solicitation will not be issued. The Offer/Quote for (RFQ)
F65501-99-Q0165 closing date is of 23 June 1999 at 12:30 PM Alaska
Standard Time. Award will be made on/or about 25 June, 1999, to the
responsive, responsible offeror whose quote offers the best value to
the government. Best value will be determined based on an integrated
assessment of each quote in terms of price, delivery, and past
performance being substancially equal. This solicitation is RESTRICTED
(1OO% Small Business) acquisition. The Standard Industrial Code (SIC)
is 7373 $18 million. The following provisions apply 52.212-1,
Instructions to Offerors, Commercial applies to this acquisition and
any addenda 52-212-3, Advising Offerors to include a completed copy of
representation and certifications -- commercial items with offer

******** Statement of Work For Database Services 1. Project Requirement
Upgrade Government owned ACM/LBP (Asbestos Containing
Material/Lead-Based Paint) database from Microsoft Access 2.0 to the
version in use on the 3 CES server (currently Access 97, SR-2). Provide
support and services identified in section 3 of this document. ACM/LBP
database was originally created by Environmental Systems, Inc., for
the Government for use at other DOD facilities. 2. General a. The
Contractor will appoint a Project Manager authorized to exercise
executive oversight of all aspects of the contract performance on
behalf of the Contractor. Any changes to the contract must be approved
by the Contracting Officer. b. The Base Civil Engineer will appoint a
Quality Assurance Evaluator (QAE). As such, the QAE will provide all
necessary technical information to the Contractor. Contracting Officer
will preside at all contract meetings with the Contractor. QAE will
verify all invoices/request for payment to the Contracting Office. c.
The Contractor will develop a work schedule within five (5) working
days after the Notice to Proceed (NTP) has been issued and submit to
Contracting Officer for approval. Once the NTP has been issued, it is
estimated that the database services will commence for a period not to
exceed I year. Liquidated damages should be assessed if work is not
completed within the performance period. d. Site visits will be
necessary to determine scope of work, coordinate with appropriate
entities, and perform installation and training services. e. While
performing services at Elmendorf AFB, work hours shall be limited to
0730 to 1630, weekdays only excluding federal holidays. f. During
performance of this project the contractor is responsible for repairing
any damage incurred to Government property due to poor workmanship, at
no cost to the government. Only the Government_s right to perform work
limits the contractors use of the premises. g. Limit use of the
premises to work in areas indicated. Do not disturb portions of the
premises beyond the areas in which the work is indicated. h. Work shall
be performed so as not to interfere with Government operations. i.
Contractor is to notify the Contracting Officer at least three days
prior to work and notification is required upon completion of the
project. 3. Scope of Work a. Contractor will upgrade Government owned
ACM/LBP database to the version of Microsoft Access currently in use on
the 3 CES server (Access 97, SR-2). Contractor will provide his own
copy of Microsoft Access software and the ACM/LBP database. ACM/LBP
database is to be re-structured to meet specific requirements for
facility maintenance at Elmendorf AFB. Specific requirements will be
identified through a site visit and confirmed by Government Official.
b. Install database on Government systems and LAN with prior approval
and coordination from computer resources and drafting personnel. c.
Perform data entry of approximately 300 records of existing samples
from hard copy files. This includes identifying material type and
condition, sample location, date, description and analytical results.
Import machine-readable data from existing database and verify data
integrity. d. Link database with AutoCAD 14 (currently used by
Government drafting personnel) to allow sample and abatement locations
to be identified in facility drawings. Provide abatement module that
tracks renovation and material disposal activities. e. Database must
have capability to download and compile data from lead sampling
performed with Niton XRF equipment. f. Track and maintain records for
personnel training, occupational physicals and respirator fit testing.
g. Provide reports that show existing ACM and LBP sample results by
building and room. h. Provide database operation manuals (15 ea.) and
training. Provide technical support via phone and e-mail. 4.
Deliverables a. Completely operational ACM/LBP database, installed on
government systems, that meets specific requirements stated insection
3. b. Database operation manuals (15 ea.) that explain proper operation
of all modules of ACM/LBP database. 5. Government Furnished Items a.
The Government will provide office space and reasonable utilities. b.
The Government will provide previous database and hard copy records for
data entry. Two computers will be made available and government will
provide technical support with LAN and AutoCAD 14. 6. Period of
Performance a. Period of performance will be approximately 10 months
from execution of the Notice to Proceed from the Contracting Officer.
