This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subparts 12.6 and 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation F41691-99Q0392 is issued as a Request for Quote (RFQ) and incorporates provisions and clauses in effect through Federal Acquisition Circular 97-11. This acquisition is for full and open competition. The government intends to award a Firm Fixed price Purchase Order under Simplified Acquisition Procedures (SAP). 12 CONS, Randolph AFB, TX has a requirement for _Management Consulting Services. For nationwide employee assistance program to provide 24-hour toll free access for recuiters and their families. Services to be provided by master_s level consultants providing personal telephone counseling, consultation and referral services for issues including stress, grief, job burnout, addiction disorders, depression, family matters, etc. Information and referral services provided to include relocation assistance, locating child care families/programs for special needs children, legal matters (divorce, landlord/tenant issues, wills/consumer issues), financial issues (credit management/budgeting, insurance /taxes), consumer services (big ticket purchases, auto/home repair), elder care options and referrals, and disability issues to include information and referral to community programs and services and caregiver support. In addition, contractor will provide management consultation services for work/employee issues to include service utilization reports quarterly, on-site critical incident response, and employee information materials to include internet access, tips on tape, library by mail/fax, and unlimited telephone consultation and personalized searches and referrals. Place of performance is FOB destination. Delivery is to begin immediately upon award. OFFERORS MUST ALSO SUBMIT A TECHNICAL PROPOSAL DESCRIBING PROCEDURES FOR CONDUCTING NATIONWIDE 24-HOUR TOLL FREE EMPLOYEE ASSISTANCE. THE GOVERNMENT WILL MAKE AN AWARD TO THE RESPONSIBLE OFFEROR WHOSE OFFER CONFORMS TO THIS REQUIREMENT AND IS CONSIDERED TECHNICALLY ACCEPTABLE, CONSIDERING PRICE AND PAST PERFORMANCE, AS FOLLOWS: 1) A DECISION ON THE TECHNICAL ACCEPTABILITY SHALL BE MADE. ONLY THOSE OFFERORS DETERMINED TO BE TECHNICALLY ACCEPTABLE WILL BE CONSIDERED FOR AWARD. 2) ALL TECHNICALLY ACCEPTABLE OFFERORS WILL THEN BE RANKED BY EVALUATED PRICE. 3) FINALLY, ALL TECHNICALLY ACCEPTABLE OFFERORS WILL RECEIVE A PAST PERFORMANCE RISK ASSESSMENT RATING OF LOW, MODERATE, HIGH OR NOT APPLICABLE. 4) IF THE TECHNICALLY ACCEPTABLE LOWEST EVALUATED PRICE OFFEROR HAS RECEIVED A LOW PAST PERFORMANCE RISK RATING, THIS OFFEROR_S QUOTE REPRESENTS THE MOST VALUABLE ACQUISITION AND AWARD SHALL BE MADE TO THIS OFFEROR. THE FOLLOWING FACTORS WILL BE USED TO EVALUATE QUOTES: (1) PRICE (2) ABILITY TO PERFORM AND (3) PAST PERFORMANCE. THE GOVERNMENT RESERVES THE RIGHT TO AWARD A PURCHASE ORDER TO OTHER THAN THE TECHNICALLY ACCEPTABLE LOWEST EVALUATED PRICE OFFEROR IF THAT OFFEROR IS JUDGED TO HAVE A MODERATE OR HIGH PERFORMANCE RISK RATING.
