Herndon, (303) 393-2858 The VISN 19 Network Acquisition Service Center
intends to award a consolidated contract for Home Oxygen Healthcare
Services to include Rental of Contractor Owned Equipment, Lease to
Purchase Equipment, supplies and services as well as, providing
delivery, servicing, and performing preventive maintenance and repair
of Government Owned Equipment for VA beneficiaries located in the
following service area; State of Colorado excluding the counties of
Montezuma, La Plata, Archuleta, and Las Animas; State of Wyoming
excluding the counties of Crook, Weston, and Niobrara; State of Montana
excluding the county of Lincoln; State of Utah; the State of Idaho
counties of Bannock, Bear Lake, Bingham, Bonneville, Caribou, Clark,
Franklin, Fremont, Jefferson, Madison, Oneida, Power, and Teton; the
State of Nevada counties of Elko and White Pine; the State of North
Dakota counties of Billings, Golden Valley, and Slope; the State of
Nebraksa counties of Banner, Cheyenne, Deuel, Garden, Keith, Kimball,
and Scott Bluff, and the State of Kansas counties of Cheyenne, Greeley,
Hamilton, Logan, Sherman, Stanton, and Wallace. The following
provisions and clauses apply plus any addenda: FAR 52.212-1
Instructions of Offerors-Commercial Items; 52.212-2, Evaluation --
Commercial Items; 52.212-3, Offeror Representations and Certifications
-- Commercial Items; 52-212-4, Contract Terms and Conditions --
Commercial Items, and 52.212-5, Contract Terms and Conditions to
Implement Statues or Executive Order -- Commercials Items. Any
additional contract requiremnent(s), or terms and conditions (such as
warranty requirements) determined by the Contracting Officer to be
necessary for this acquisition and consistant with customary commercial
practices, will apply. All equipment, supplies, and services will be
provided to the VA beneficiaries residence. This will include set-ups,
exchanges, service and repairs, monitoring visits by a Registered
Respiratory Therepist (RRT) or a Certified Respiratory Therapist (CRT).
This will include a 24 hour -- 7 day a week service scenario including
all Government recognized holidays. The provider will be accredited by
the Joint Commission of Acceditation of Health Care Organization
(JCAHO) and in compliance with all current recognized and required
standards. Acceditatation shall be for both Clinical Services and
Equipment Services. Compliance is required with all
regulations/requirements with the Department of Transportation (DOT);
Compressed Gas Association (CGA); National Fire Protection Agency
(NFPA); Federal Drug Administration (FDA), and the Occupational Safety
and Health Administration (OSHA). The Government is interested in a
single provider for the entire geographic area described herewith
however all qualified offers will be welcomed. The Contractor will have
the ability to provide access to Government Owned VisTa Database
systems which provide support to P&SAS record patient information,
invoicing, and other clinical and administrative applications. The
Standard Industrial Code is 8082(business size: 5.0 Million in gross
annual sales for the last 3 years). Contract term will be for Base Year
with four one-year options for renewal. Interested parties are invited
