Provide Physical Therapy services for the Coast Guard clinic located at Sitka, AK. Patients will be referred for treatment by a Coast Guard Medical Officer to the contractors facility. This will be a non-personal service contract and medical liability insurance in the amount of $1.0 million will be required. This award will be accomplished utilizing Best Value Commercial Item procedures. The contractor shall normally work a maximum of 8 hours per week (1 day) on weekdays, not to exceed 416 hours in the base year and 416 hours in any option year. Period of performance will be 01 October 1999 through 30 September 2000 with three option years.
