The PVAMC is looking to contract with a vendor located within three miles of the Central Oregon VA Primary Care Clinic (COVAPCC) located at 2042 Williamson Professional Park, Bend, Oregon, for prescription filling services (first fill) for acute conditions deemed to be urgent by the COVAPCC. The contractor will provide prescription screening, dispensing, and counseling services (by a registered and licensed pharmacist). VA eligible beneficiaries shall present an original prescription (VA Form 10-2577d) to the Contractor. All prescriptions shall be filled for the quantities as written, not to exceed a 15 day supply with the exception of those products that the manufacturer's package size prohibits dispensing in a smaller quantity (e.g., inhalers, insulin vials). Contractor's staff and facility must meet all applicable federal and state requirements for dispensing pharmaceuticals. Evaluation criteria are described in the solicitation. It is estimated that a contract offer will be made by August 11, 1999 with services to begin August 16, 1999. This is a commercial item request for quotation. Contract offer will include a base year with a one-year option to extend. All quotations must be received in the issuing office no later than 4 p.m. July 21, 1999. All interested parties are invited to submit a quotation.
