LINE ITEM 1: Service Contract Option 1 -- Qualified technician from RP Adams Company to evaluate the control panel and determine what is needed to repair the control panel to its proper function. Technician will itemize control panel work and cost of repair shall be submitted to Government for evaluation. Government will determine if technician will proceed with the repair or if a new panel is warranted. LINE ITEM 2: Service Contract Option 2 -- Cost to furnish and install new control panel at Rainbow Village Site, San Juan County, Utah, shall also be presented by RP Adams. LINE ITEM 3: Manuals -- Prepare and deliver 2 sets of step-by-step operations and maintenance manuals for the De Filter System. LINE ITEM 4: Training -- Provide in-depth operator training session. LINE ITEM 5: Start-up and Test -- After all new and/or reconditioned equipment for the De Filtration System has been installed, provide start-up and testing services for the entire system consisting of control panel, body feed tank and feed tank mixer. LINE ITEM 6: Travel and Per Diem -- All travel costs to and from site of treatment system, including per diem and air fare, Government rates are applicable for all lodging and meals. The government rates for Page, Arizona are Lodging -- $67 per day, meals -- $34 per day -- The Government rates for Kayenta and Tuba City, Arizona are Lodging -- $92 per day, Meals $30 per day. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302. This notice of intent is NOT a request for competitive proposals; however, all proposals received within 45 days (30 days if award is issued under an existing basic ordering agreement) after date of publication of this synopsis will be considered by the Government.
