The Albuquerque Area Indian Health Service will be soliciting for proposals from prospective offerors for psychological services to be rendered to the patients served by the Albuquerque Indian Hosptial, 801 Vassar Dr., NE, Albuquerque, NM 87108. This acquisition is set aside as a total 100% small business set-aside. Contractor to provide as such: 1. Contractor will provide 600 hours per year of services in their own office/clinic, and 2. contractor will send a professional consultant to two pueblo communities within the Albuquerque Indian Hospital service area for 76 full clinic days of service. The two pueblos are Isleta Pueblo, Isleta, NM and Jemez Pueblo, Jemez, NM. The contract will be awarded as a single contract for a base year with four, one-year options for renewal. Contractor must have on his/her staff; a doctoral level licensed Physologist, Master's level licensed social workers and counselors, and a licensed Child Psychiatrist. Majority of contractor's staff must have over two years clinicalexperience, training and specialized skills to treat multi-cultural children and families. All staff must have training in child, family and couples therapy. Contractor's facilities will have a child centered therapy/treatment rooms with games, sand boxes, toys, small furniture and conditions conducive to the comfort of children. The contractor's clinic/facility will be centrally located in urban Albuquerque and easily accessible by city bus service. All propoals received will be evaluated and considered. The technical and cost/pricing proposals are of equal value. In the case of technical proposal tie, cost factors will determine the award of a contract.
