Q -- PHLEBOTOMIST SOL RFQ 124-0020 DUE 082099 POC Richard Houser, Contract Specialist 608-584-6120 The Federal Correctional Institution, Oxford, Wisconsin is soliciting to establish a non-personal service agreement to provide the services of a Phlebotomist. The contractor shall perform basic laboratory procedures in-house to include: dipstick urinalysis, occult bloods by Hemoccult, blood sugars by Accu Check III glucometer, pre and post HIV test counseling, scheduling of patients for laboratory testing, and filing of lab reports in individual medical records. Also, the maintenance of daily laboratory log and laboratory files and performance of daily urine and Accu Check III controls, specimen collection and preparation of specimens for shipment to reference laboratories. Contractor will provide written reports of laboratory tests performed on all laboratory specimens performed at the F.C.I. and F.P.C. Contractor is required to perform all testing commensurate with the College of American Pathologists, CLIA-88, and JCAHO standards. Contractor will furnish copies of all state licenses, diplomas, boardcertifications, a curriculum vitae, any continuing education received during the contract period, and proof of professional liability insurance for credentialing pursuant to JCAHO standards and Federal Bureau of Prisons policies. Contractor will provide fourteen, two hour sessions per week, Monday through Friday for a total amount of 732 sessions for the expected contract period of October 1, 1999 through September 30, 2000. An option year contract is contemplated, cost will be based on the total price for the basic requirement together with any option(s) exercised at the time of award. This acquisition is 100% set-aside for small business. All responsible sources may submit a quotation which will be considered by this office.
