Contracting Officer The Federal Correctional Institution, Oxford,
Wisconsin is soliciting to establish a non-personal service agreement
to provide the services of an Optometrist. The Contractor shall render
care, treatment, and evaluation of the ocular needs of the inmate
population. The Contractor shall also provide routine optometric
examinations including refractions, tonometry, and the prescribing and
dispensing of eyeglasses. Additionally, visual field, routine
diagnostic ocular and fundoscopic examinations will also be performed,
as requested. The Contractor will have eyeglasses available for
patient purchase, if the patient's choose eyeglasses other than the
Unicor eyeglasses provided to them at no charge. Contractor must be a
graduate of an approved school of Optometry with specialty training and
licensure for prescribing therapeutic agents in the treatment and
diagnoses of ocular disease(s) and must possess current state
certification and/or licensing in the State of Wisconsin for Optometry.
Contractor will furnish copies of all state licenses, diplomas, board
certifications, any continuing education received during the contract
period, and proof of professional liability insurance for credentialing
pursuant to JCAHO standards and Federal Bureau of Prisons policies.
Contractor will provide two hours per session, four sessions monthly
with four additional sessions every other month, Monday through Friday
for a total amount of 76 sessions for the expected contract period of
October 1, 1999 through September 30, 2000. An option year contract is
contemplated, cost will be based on the total price for the basic
requirement together with any option(s) exercised at the time of award.
This acquisition is 100% set-aside for small business. All responsible
sources may submit a quotation which will be considered by this
office. The estimated range of this agreement is $25,000 to $100,000.
