D -- PAGER LEASING AND AIR TIME SERVICES SOL RFP 673-12-00 DUE 092099 POC Debra Dodson (813) 972-7588 This is a combined synopsis/solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Request for Proposal (RFP) number 673-12-00 applies. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-12. Provide pager services for the VA Outpatient Clinic, Orlando, Florida, 5201 Raymond Street, Orlando, Florida 32803. This proposal, if accepted, shall be effective for the period of October 1, 1999 through September 30, 2000. The Government may extend the term of this contract or four additional option years, duration not to exceed September 30, 2004. Service shall include: immediate replacement of malfunctioning or inoperable pagers. 24-hour emergency on-call service. Responsibility fornotifying the VA when paging system and/or associated equipment is inoperable for more than one (1) hour which would cause lack of signal transmission or paging capability. Offeror shall submit with proposal a Conversion Plan to assist in the conversion of existing pagers to offeror's system. A Disaster Contingency Plan outlining how service shall be continued for medical response team pagers shall be made available upon request. Pricing shall include: lease, airtime, unlimited calls, batteries, belt clips, battery covers, and delivery. Price shall be submitted in terms of monthly cost per unit and monthly total for each line item: 1. Estimated 28 new "Motorola Pronto Flex Digital Tone Pagers or equal" with voice mail retrieval. 2. Estimated 77 new "Motorola Pronto Flex Digital Tone Pagers or equal" with numeric display. 3. Estimated 17 new "Motorola Alphanumeric Advisor pagers or equal". Services for the Alphanumeric pagers are to include, but not be limited to the following: Response time of thirty (30) toforty-five (45) seconds or less, unlimited calls with seven day, 24-hour statewide coverage, use of two (2) "Motorola Alphamate Keyboards or equal". Pricing shall also be submitted for each additional option year. All proposals will be evaluated. The Government intends to make a single award to the responsible offeror whose offer is the most advantageous to the government considering factors contained herein. Provision 52.212-2 Evaluation Commercial Items, applies with paragraph (a) completed as follows: "The following factors shall be used to evaluate offers: (1) Past performance (2) Technical capabilities (3) Pager replacement plan (4) Price. Past performance, Technical capabilities, and Replacement plan when combined are more important than price. Offerors shall provide all necessary documentation needed to support evaluation factors listed above, to include paging system, transmission frequency and speed (response time) for each type of pager.
