is a combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR, Subpart 12.6 as supplemented with
additional information included in this notice. This notice constitutes
the only solicitation; request for proposals are being requested and a
written solicitation will not be issued. This solicitation is being
issued as RFP 688-1-00. The solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular number 97-11. Any amendments to this solicitation will be
published only in the CBD. The SIC code for this solicitation is 7699
with total revenues not to exceed $5.09 million. Contractor shall
furnish all labor, tools, personnel and equipment necessary to provide
preventive maintenance/service on Model #MACM10 Electron Microscope,
serial #D838, Gatan TV System -- Model# MA5201, Serial# D838.1 Contract
is for the period of 10/1/99 thru 9/30/2000 with two (1) year renewal
options, subject to the availability of funds. Services shall be
provided at the Department of Veterans Affairs Medical Center, located
at 50 Irving Street, Washington, DC 20422 in accordance with all terms
and conditions of this solicitation. Contractor shall provide a time
and materials hourly repair rate including any associated travel costs
for the repair of above items. Coverage is required from 8:00am to
5:00pm, Monday thru Friday, excluding government holidays. Contractor
shall provide call-back confirmation within twenty-four (24) hours and
shall have a service engineer on site within four (4) business days of
a reported problem. The contractor shall be responsible for protecting
the personnel furnishing services under this contract. To carry out
this responsibility, the contractor shall provide the following for
these personnel: workers compensation, professional liability
insurance, health examination, income tax withholding, and-social
security payments.The parties agree that such personnel shall not be
considered VA employees for anypurpose and shall be considered
employees of the contractor. The following provisions apply to this
solicitation: 52.212-1 Instructions to Offerors-Commercial, 52.212-3
Representations and Certifications-Commercial Item, 52.212-4 Contract
Terms and Conditions -Commercial Items, and 52.212-5 Contract Terms and
conditions including clauses incorporated by reference under Section
(b) (6) 52.222-26, Equal Opportunity (E.O. 11246, Affirmative Action
for Special Disabled and Vietnam Era Veterans (38 U.S.C. 4212),
52.222-36 Affirmative Action for Handicapped Workers (29 U.S.C. 793),
Buy American Act-Supplies (41 U.S.C.10) A written notice of award or
acceptance of offer, mailed or otherwise furnished to the successful
offeror within the time for acceptance specified in the offer shall
result in a binding contract without further actions by either party.
Offerors responding to this announcement shall submit a price for each
category as indicated along with the information required in 52.212-1
and a completed copy ofprovision 52.212-3 to VA Medical Center
(688/90c), Room 1B112, 50 Irving Street NW, Washington, DC 20422 by
August 13, 1999 at 4:30 pm local time. Faxed offers will be accepted.
