Mountain Network Acquisition Service Center anticipates an award for a
combined VISN (19 and 21) consolidated contract for Home Oxygen
Healthcare Services which includes Rental of contractor owned
equipment, lease to purchase oxygen equipment, Rental or purchase of
oxygen equipment, supplies and services, home delivery, performing
preventive maintenance, servicing and repair/parts replacement
including Government owned equipment for veteran beneficiaries located
in VISN 19 providing coverage in the State of Colorado excluding the
counties of Montezuma, La Plata, Archuleta, and Las Animas; State of
Utah; State of Montana excluding the county of Lincoln; State of
Wyoming excluding the counties of Crook, Weston, and Niobrara; State of
Idaho, the counties of Bannock, Bear, Lake, Bingham, Bonneville,
Caribou, Franklin, Fremont, Jefferson, Madison, Oneida, Power and
Teton; State of Nevada, the counties of Elko and White Pine; State of
North Dakota, the counties of Billings, Golden Valley, and Slope; State
of Nebraska, the counties of Banner, Cheyenne, Deuel, Garden, Keith,
Kimball, and Scott Bluff; State of Kansas, the counties of Cheyenne,
Greeley, Hamilton, Logan, Sherman, Stanton, and Wallace. Also for
veteran beneficiaries located in VISN 21, providing coverage in the
State of California, all the counties north of the county lines of San
Luis Obispo, Kern, and Inyo excluding the county of Del Norte; State
of Nevada, the counties of Washoe, Humboldt, Lander, Eureka, Churchill,
Mineral, Douglas, Storey, and Lyon; State of Hawaii. The following
provisions and clauses apply plus any adenda: FAR 52.212-1 Instructions
of Offerors-Commercial Items; FAR 52.212-2 Evaluation-Commerical Items;
FAR 52.212-3 Offeror Representations and Certificatins-Commercial
Items; FAR 52-212-4 Contract Terms and Conditions-Commercial Items, and
FAR 52.212-5 Contract Terms and Conditions to Implement Statues or
Executive Order-Commercial Items. Any additional contract
requirement(s), or terms and conditions (such as warranty requirements)
determined by the Contracting Officer to be necessary for this proposal
and consistant with the customary commercial practices, will apply. All
equipment, supplies, and services will be provided to the veteran
beneficiaries residence. The VISN 19/21 consolidation for these
resources has a primary interest in a single provider for the
geographic area's described herewith, however all offers will be
accepted. The Standard Industrial Code is 8082 (business size: 5.0
Million in gross annual sales for the last three years). Contract terms
will be for a Base Year with four one -- one year options for renewal.

Proposals will be issued August 20, 1999. Prior requesters responding
to notice number 344650, June 18, 1999, NEED NOT REAPPLY, proposals
will be automatically shipped to the addresses that have already been
submitted to this office. Solicitation Number 344650 is hereby canceled
-- please refer to all directions and specifications listed in this
notice for the supplies, services, etc., listed in the prior notice.
