The successful offeror will be required to provide a team of professionals, including Board Certified nephrologist(s), registered nurse(s), licensed practical nurse(s), nurse aid(s), social worker(s), and dietician(s) to administer hemodyalisis services on an outpatient basis at the contractor's facility, and an inpatient basis at the VAMC. Requirements are per procedure and not on an FTEE or Locum Tenens basis. The period of performance for this contract will be from October 1, 1999 through September 30, 2000 with 2 additional years at the option of the government. The applicable SIC Code is 8092.
