Procedures will include: (1) Comprehensive Eye Exam to diagnose or confirm the presence of cataract and to determine need for surgery. (2) Cataract Surgery to include pre-operative testing, extracapsular Cataract Removal with insertion of intraocular lens prosthesis,(Unilateral). Local anesthesia and monitoring during surgery, outpatient surgery facility services to include Intraocular lens, postoperative services to include 90 days postoperative follow-up care. (3) Discission of secondary membraneous cataract (if needed). Ophthalmologist must be board certified in Ophthalmology and proof of certification and license must accompany offer.
