R -- SUPPORT ACTIVITIES FOR HRSA/BPHC'S NATIONAL DIABETES PARTNERS PROGRAM SOL HRSA-99-BPHC-0118 DUE 082999 POC Aileen Broider-301-443-5231 The Health Resources and Services Administration intends to negotiate on a SOLE SOURCE basis with Robert Rolfsen, 4523 Drusilla Drive, Baton Rouge, LA 70809. This procurement shall be processed in accordance with the Simplified Acquisition Procedures in Part 13 of the Federal Acquisition Regulations. The purpose of this procurement is to support activities directly related to the Bureau of Primary Health Care's National Diabetes Partners Program (BPHC NDPP). The contractor shall: develop and implement a plan to evaluate the impact and/or outcome of BPHC NDPP activities; develop, expand and coordinate planning and activities of the BPHC NDPP; attend meetings of the BPHC NDPP and provide support; publicize accomplishments of BPHC NDPP efforts at national, regional and/or State meetings; coordinate efforts to foster and create community diabetes coalitions in each community where BPHC supported health centers are located; and prepare a final report of all project activities. Robert Rolfsen is deemed the only responsible source capable of providing the required work and services. The period of performance is one year.
