is provided on a sole source basis, with intent to award a firm fixed
order with UCSD Medical Genetics. This is a combined
synposis/solicitation for commerical items, prepared in accordance with
the format Subpart 12.6 as supplemental with additional information
included in this notice. This announcement constitutes the only
solicitation; other interested parties may submit proposals to the
address indicated above Attn: Sharon Moore. A written solicitation will
not be issued. The solicitation, number N00259-99-T-0130, is issued as
a Request for Quotation (RFQ). The solicitation document and
incorporated provisions and clauses are those in effect through FAC
97-11. The standard industrial classification (SIC) code for this
acquisition is 8071. Calls will be placed from 01 Sep 99 through 30 Sep
99 against Blanket Purchase Agreements (BPA's), however, the exact
number of tests cannot be pre-determined. The number of required tests
may fluctuate depending on patient care. The types of laboratory
services required under this effort will include: amino
karyotype/chromosome amniotic fluid, blood chromosome, bone marrow
chromosome, DNA Fragile X, tissue chromosome. Contractor must provde to
the Naval Medical Center, San Diego on site access to consultants who
participate in genetic counseling and specimen collection. In addition,
contractor may be used on a consult basis in those patient care cases
where specialized equipment is needed and will be used on their
premises. Lab testing must be performed Monday through Friday and the
contractor will be required to provide lab results to the Naval Medical
Center, San Diego by means of either facsimile, mail, or contractor
provided printer. This acquisition incorporates the following FAR
clauses: 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERICAL ITEMS (Oct 95);
52.212-2 EVALUATION-COMMERICAL ITEMS (Oct 95) is incorporated by
reference with the following addenda; in paragraph (a) the government
will evaluate proposals based on past performance, technical capability
and cost to the government. All other factors when combined, make up 50
percent of evaluation factors as compared to price; 52.212-3 OFFEROR
REPRESENATIONS AND CERTIFICATION-COMMERICAL ITEMS (Oct 95) must be
completed and submitted along with the proposal (a copy can be obtained
by contacting Sharon Moore at (619) 532-9202; In accordance with FAR
clause 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT
STATUTES OR EXECUTIVE ORDERS-COMMERICAL ITEMS ((Oct 95), the following
clauses are hereby incorporated by reference in paragraph (b)

52.222-37 EMPLOYMENT REPORTS ON SPECIAL DISABLED VETERANS AND VETERANS
OF THE VIETNAM ERA (38 U.S.C 4212). Funds subject to the enactment of
the FY98 DOD appropriation act or the FY97 continuing resolution
authority. All provisions apply of whichever act becomes available.
GFAR Clause 52.232-18 (Availability of Funds April 1984) applies.
