This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. This is a simplified acquisition, issued as a Request for Quotation (RFQ), number SSA-RFQ-99-3972. This request for quotations and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-12. This requirement is 100% set-aside for small businesses only. All responsible small business sources may submit a quote which shall be considered by the agency. Quotations received from other than a small business will be deemed unacceptable and will be given no further consideration. The standard classification code for this requirement is 3663. REQUIREMENT: The Social Security Administration in Woodlawn, MD requires delivery of the following communications equipment: (1) Vertex Portable Radio, Model VX-10 with 16 button keypad; includes antenna, battery, belt clip, 1 hour desktop charger, Quantity: 25 each; (1)(a) Set-up/Program Test for each radio, Quantity: 25 each; (2) Vertex FNB-47 7.2v 1100 MAH NI-CAD Battery 5-WATT, Quantity: 25 each; (3) JPS Signal-to-Noise Voting System. Configured for full 12-channel with voted site talk-through and enclosed in secured table top cabinet model #SNV-12, Quantity: 1 each; (4) Engineering and Consulting Fees required for installation, testing and training, Quantity: 1 each; (5) Model VSIM-4 communications Voter Site Interface Module. Radio Receiver/Transmit Interface with two (2) connections. Require 120VAC and modular patch cord. Includes two (2) spares, Model #FTL-7011, Quantity: 14 each; (6) Installation fee/set-up/operational testing, all locations and personnel training, Quantity: 1 each. Due to the fact that SSA already has quantities of this exact communications equipment already in house, the quoter shall quote on the brandname product only. All equipment shall be shipped and all quotations shall be submitted on an F.O.B. destination basis only. The ship-to address is Social Security Administration, Attn: Receiving Clerk, 6401 Security Blvd., Baltimore, MD 21235-6401. In its quotation, the quoter shall include the following information: business size; does the quoter accept the Government bankcard; prompt payment terms if the quoter does not accept the Government bankcard; time of delivery in calendar days; federal tax number; DUNS number; company name, address and phone number as it shall appear on the award document. The Government's purchase order number, 0440-99-37934, must appear on all boxes, cartons and containers delivered. BASIS FOR AWARD: Award will be made to the quoter whose quote represents the technically acceptable, lowest overall total price to the Government. The quote shall be deemed as unacceptable if the quoter fails to quote on all of the listed line items. The Government shall make only one (1) award in the aggregate. The provisions at FAR Part 52.212-4, Contract Terms and Conditions Commercial Items apply to this acquisition. There are no addenda to these provisions.
