Amarillo VA Health Care System, Amarillo, Texas has a requirement for
a Phacoemulsification Unit with the following speicifications: --
Ultrasonic hydrodelineation with 29 gauge ultrasonic needle. -- Ability
to drive ultrasonic handpiece frequencies, 20 kHz to 80 kHz. --
Ultrasonic tip styles to include round, epsilon, microtip, kelman,
mackool and ABS tip technology. Tip design must increase cutting
efficiency and reduce positive pressure and turbulence. -- Ultrasnoic
control: Must offer either surgeon, panel, or pulsed control and
encompass a minimum of 1,125 linear steps of resolution power. --
Peristaltic aspiration: Pump design must allow smooth linear vacuum
from 0 mmHg to maximum preset and greater chamber stability. --
Ultrasnoic vacuum: 0 -- 500 mmHg. -- I/A: Surgeon or panel control of
vacuum and aspiration, linear vacuum 0-600 mmHg and linear aspiration
flow rate 0-60 cc/min. -- Surgeon control of aspiration and vacuum:
Must allow linear control of AFR pump speed and vacuum limit in foot
position 2, up to the preset panel setting. -- Panel control and
Surgeon control of burst phaco: Control of ultrasound power width from
5-500 milliseconds in increments of 5 milliseconds in surgeon control.
-- Foot pedal: Must allow control of aspiration flow rates which can be
independently set for I/A and phacoemulsification foot positions, flow
rates must be from 0-60 cc/min. -- Capsule vacuum mode: Preset vacuum
pressure and aspiration rate for vacuuming the posterior capsule. --
Microprocessor controlled fluid venting: Must ensure rapid and
controlled venting for improved chamber stability. -- Automated reflux:
Continuous reflux of irrigation from infusion line activated from
footswitch. -- Anterior vitrectomy: Includes I/A cutter probe which
allows adjustable port and adjustable cutting rates and is controlled
by footswitch and allows (1) irrigation, (2) irrigation, cut, (3) cut.
-- Cassette System: One piece cassette with integrated fluidic
channel.—Bipolar coagulation: Allows surgeon or panel control of
diathermy. -- Wireless remote control: Full function. -- Unit must be
programmable for surgeon preferences. -- Fully programmable
footswitch. -- Automated I/V pole: Control through panel or footswitch.
-- Voice confirmation: Confirms mode selection with voice feedback. --
Color touch screen, tiltable and rotatable: One touch access, 256
color monitor LCD screen. -- Set-up: Automated priming and depriming,
with self diagnostics, for ease of use by nursing staff. --
Microprocessor controlled: Must have 3 independent microprocessors for
fluidics, ultrasonics, and front panel user interface. -- Must be
upgradable and Y2K compliant. -- Service: On-site direct field service
engineer with 24 hour response time. -- System must be UL544 and
IEL-601 approved. -- Delivery: System must be delivered within 45 days
after receipt of order. -- Evaluation Factors: The following
evaluation factors will be applied: -- Price (30%)—Technical
Factors (30%) A. Compliance with technical specifications B. Ease of
Use (User friendly options) -- Quality -- Past Performance (40%) The
following items must be submitted with each offer: A. Price proposal.
Evaluation will be made on the total price for the base unit and all
options required by this specification. B. Technical proposal. Must
address technical evaluation factors by submitting a comprehensive
technical fact sheet. C. Quality proposal. Must submit information to
demonstrate a minimum of 5 years of performance in this field and
include a list of references, with names and phone numbers, for sales
of like equipment. Other VA and Government sales are preferred. 2. This
is a combined synopsis/solication for commercial items prepared in
accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. 3. To be delivered to the
Amarillo VA Health Care System, 6010 Amarillo Blvd., West, Amarillo,
Texas 79106. 4. Provision at 52.212-1 is included and has been modified
to include evaluation factors. 5. Due date for proposals is 8/31/99,
