This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation, number N00259-99-Q-2016, is issued as a Request for Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-12. This request is for a Service Maintenance Contract for a Del Mar Holter System, Model 563, Serial Number 472 to include two (2) preventive maintenance inspections, parts, labor, transportation and unlimited corrective maintenance. Period of performance is from 01 Oct 99 through 30 Sep 00 (Base Year Year, line item 0001). This requirement is to include four (4) option years: Option Year One from 01 Oct 00 through 30 Sep 01; Option Year Two from 01 Oct 01 through 30 Sep 02; Option Year Three from 01Oct 02 through 30 Sep 03; and Option Year Four from 01 Oct 03 through 30 Sep 04.
