This is a combined synopsis/solicitation for a commercial item prepared
in accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. IMPORTANT NOTICE: FAR
52.232-33 (Payment by) Electronic Funds Transfer ( -- Central
Contractor Registration) requires registration in the CCR database.

MANDATORY TO OBTAIN A DUNS NUMBER PRIOR TO REGISTERING. CALL DUN &
BRADSTREET (1-800-333-0505) TO OBTAIN. AWARD CAN NOT BE MADE UNLESS
REGISTERED. Solicitation No. F08650-99-T-3398 is issued as a Request
for Quotation. The solicitation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
97-12 and through Defense Federal Acquisition Regulation Supplement DCN
1990804. This solicitation is 100% set-aside for small business. It is
advertised with a Standard Industrial Classification Code (SIC) of
8049, with a size standard of $5 million. Contractor shall furnish all
labor, material, music, transportation, and all other items necessary
to provide training on proper nutrition for 10 to 30 personnel that
will be held in Bldg. 546, Health and Wellness Center, on Patrick AFB,
FL from 01 Oct 99 -- 30 Sep 00. Work schedule dates and times to be
mutually agreed upon. Variation in locations will be coordinated
fifteen (15) days in advance and remain within Brevard County, Florida.
Following is a list of program requirements and hours of services

$____________. PROVIDE PRICING FOR BASIC YEAR AND OPTION YEARS I
THROUGH III (LINE ITEMS 0001 -- 0004). BIDDERS MUST BID ON ALL ITEMS
(BASIC PLUS OPTION YEARS) TO BE CONSIDERED FOR AWARD. The list of
course material is too diverse and lengthy to provide however, the
following is an overview of programs offered: 1. Disease management
includes a variety of program topics including but not limited to: a.
Wellness Clinic -- a six-week multidisciplinary program designed to
provide beneficiaries with necessary tools to manage their weight
through lifestyle changes. b. Diabetes Management Program -- a six-week
program designed to provide patients with an understanding of how to
manage their disease by understanding the nutritional, exercise and
pharmacological concepts of treatment. This program is in the process
of being certified and must be taught by a Certified Diabetes Educator.
An eight-hour survival program is also taught each quarter. c. Lipid
Management Program -- a multi-session program designed to target people
with hyperlipidemia and to educate patients so they gain an
understanding of the disease process and how to manage their lipid
levels through nutritional and exercise efforts. d. Osteoporosis
Program -- provide patients with an understanding of the nutritional
aspects of this disease. e. Prenatal Education -- Provide nutritional
information on a recurring basis to beneficiaries identified as
pregnant. f. Hypertension -- Provide nutritional education information
for individuals diagnosed as hypertensive. g. New nutritional
educational programs as the mission requires. 2. Sensible Weigh -- A
four-week program specifically designed to provide active duty
personnel with a lifestyle change approach to personal weight
management. 3. Nutritional Education/Counseling -- the nutritional
education component of the Health and Wellness Center is designed to
provide comprehensive dietary assistance to our beneficiary population.
The objectives of our many nutritional programs are: -To manage the
nutritional aspects of the Air Force Weight Management Program. -To
educate Patrick AFB personnel about the importance of sound nutrition
practices. -Encourage and assist beneficiaries to establish and
maintain dietary habits that contribute to good health, disease
prevention and weight control. -Assist in modification of the base
environment such that it encourages and supports healthy eating habits.
-Supports the MTF providers and TRICARE network providers as a source
for nutritional consultation. The following provisions and clauses
apply to this acquisition: FAR 52.212-1 Instructions to Offerors --
Commercial Items is hereby included by reference. The following are
addenda to FAR clause 52.212-1; At paragraph (c), Period of Acceptance
for Offers, the amount of time an offeror must hold its prices firm is
hereby changed. An offeror must hold the prices in its proposal, firm
for 60 calendar days from the date specified for receipt of proposals.
Paragraph (e) multiple offers has been determined by the Contracting
Officer to be non-acceptable to this solicitation and is hereby removed
in its entirety for the purpose of this requirement. Offerors are
advised that award may be made without discussions. Award might be made
to the responsible offeror whose proposal is responsive to the terms of
the RFQ and is most advantageous to the government. In accordance with
FAR subpart 9.104-1, each offeror is required to provide information
on a reasonable number of recent and relevant contracts for the same or
similar items, as well as the offeror's primary financial institution,
to include contract numbers, points-of-contacts, telephone numbers,
and any other relevant information, which will be used by the
Contracting Officer to make a responsibility determination about a
prospective offeror. Far provision 52.212-2 Evaluation -- Commercial
Items applies to this solicitation and the following evaluation
criteria (arranged in their relative order of importance) are hereby
incorporated: Award will be made to the offeror who meets the
solicitation's minimum criteria for technical acceptability with
regards to past performance at the lowest price. To be determined
technically acceptable, the offeror must show proof they are a
registered dietitian and certified diabetes educator (ADA). FAR
provision 52.212-3 Offeror Representations and Certifications --
Commercial Items shall be completed and submitted with the proposal.
FAR clause 52.212-4 Contract Terms and Conditions -- Commercial Items
hereby applies to this solicitation. The following FAR Clause is hereby
incorporated by reference: 52.232-18 Availability of Funds. FAR clause
52.212-5 Contract Terms and Conditions Required to Implement Statutes
or Executive Orders -- Commercial Items hereby applies to this
acquisition. In accordance with FAR clause 52.212-5, the following
clauses are hereby incorporated by reference: 52.217-8 Option to Extend

submit their quote in accordance with their standard commercial
practices but must include the following information: Company's
complete mailing and remittance address, discounts for prompt payment,
if any (e.g. 1% 10 days), the company's CAGE Code, Dun & Bradstreet
number, and Taxpayer ID number. The government intends to make a single
award to the responsible offeror whose offer is the most advantageous
to the government considering price and price-related factors. Offers
