This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is F64133-99-QA004, and is issued as a Request for Quotation (RFQ). Request for Quotation (RFQ) to provide all parts, labor, material, equipment and transportation to provide Aerobic Instruction Services as specified in the Statement of Work. This solicitation document and incorporated provisions and clauses are those in effect as of Federal Acquisition Circular 97-12. This requirement is a total Small Business set aside. The Standard Industrial Classification (SIC) Code is 7999, and the small business size standard is $5.0 mil. The solicitation will be written in accordance with Federal Acquisition Regulation (FAR) Part 12 for Acquisition of Commercial Items. The Request for Quotation (RFQ) will be available via the Internet at the following website: www.eps.gov. Please ensure your fax proposals are marked with RFQ number, closing date and time. Offers from sources on Guam shall include Guam Gross Receipts Tax (GRT) in their unit prices. It is not necessary to submit product samples and commercial specifications with the proposal.
