Subpart 12.6, as supplemented w/additional information included in this notice. This a notice constitutes the solicitation; quotes are being requested; separate solicitation document won't be issued. (ii) Solicitation number is DE-RQ08-99NV13738, REQUEST FOR QUOTATION (RFQ). The solicitation document and incorporated provisions/clauses are those in effect through Fed. Acquisition Circular 97-12. (iv) This requirement is 100% set-aside for small businesses. Standard Industrial Classification Code is 1731. Size standard is $7M (avg. of total sales over 3 years can't exceed the size standard). (v) Offeror shall provide all materials, labor, and transportation necessary to provide 12 months of electrical preventative maint. for the Nevada Support Facility (NSF), 232 Energy Way, N. Las Vegas, NV 89030. (vi) Following is statement of work (SOW) for electrical preventative maint.: Offeror is to perform biannual testing and maint. on transformers. Annual testing & maint. is to be performed on switchgear & switchgear connectors, switchboard assemblies, protective relays, metering systems, personnel protective grounding system, electrical equip. neutral grounding systems, capacitors, lightning/surge arresters, surge protection devices & routine inspections, testing & servicing of electrical panels to include: loose wires & lugs, heaters, lugs & indicating lights, all emergency ballasts for proper function, all lighting contactors, & transformer connections. First biannual testing & maint. of transformers & all annual testing & maint., except switchgear and switch gear connectors, are to be completed within 4 months of award of Purchase Order (PO). Second biannual testing & maint. of transformers is to occur approximately 6 months after first test. Annual testing & maint. of switchgear and switch gear connectors should occur between July & Sept. 2000. Servicing of transformer connectors, switchgear connectors, and all scheduled or potential disruptions of power shall be coordinated thru designated DOE/NV Point of Contact (POC) prior to that work being performed. This PO will include testing/inspection of following equip.: 7 -- 112.5 KVA transformers; 60 electrical circuit breaker panels; 3 -- 75 KVA transformers; 1 Cutler-Hammer by-pass maint. switch; 2 -- 45 KVA transformers; 1 Cutler-Hammer by-pass isolaton switch; 6 -- 30 KVA transformers; 1 GE AV-line switch board (480v); 2 -- 15 KVA transformers; 8 GE power panels; 2 -- 75 KVA power conditioners; 16 feeder breakers; 2 -- 50 KVA power conditioners; 2 main breakers; 1 -- 30 KVA power conditioner; 1 tie breaker; 5 transient voltage surge suppressors; 8 GE 8000 Line Control Centers (480 amp); 1 GE 300 Line Control Unit; 19 GE Heavy Duty Safety Switches (30 amp); 2 GE Heavy Duty Safety Switches (400 amp); 1 emergency shut-down panel; 1 heavy duty safety switch (100 amp); 1 line control unit (480v); 1 -- 480v circuit breaker; 2 GE Distribution/Control Units Substations consisting of master unit, heater & light switches, a GE AKD-8 switchgear, main breaker and secondary disconnect; 2 MGM transformers (480v); 1 Power con Corp Transition Unit; 4 Power Con Corp 15 KVA 600 amp Model S-12241, D-9512 Load Interrupters; 1 Power Con Corp Metering Unit. In addition to biannual & annual inspections, all exterior lamps located on the perimeter of the building & associated grounds are to be checked monthly. Replace as required. (All lamps shall be provided by DOE/NV from warehouse stock and DOE/NV shall dispose of all lamps.) Documentation & use of transformer test report form, switchgear checklist form, infra-red scan report form, air circuit breaker test & inspection report form, disconnect switch test and inspection report form, protective relay test report, current transformer (ct) test record, potential transformer (pt) test record, & dielectric test sheet shall be required for this preventative maint. contract. Offeror shall provide a written report of potential failure conditions at end of each inspection. (see website for forms). NSF is a secured facility & requires DOE/NV sign-in and/or escort for any work to be done. Coordination w/POC is required prior to arrival to perform work to avoid delays in gaining access to NSF. If access is denied due to failure to coordinate arrival w/POC, the Gov't won't be liable for costs of that trip. (vii) Delivery Point is NSF, 232 Energy Way, N. Las Vegas, NV 89030. Acceptance is at destination. Delivery is per instructions in para. (vi). NOTE: Following clauses are located at website: (viii) FAR provision 52.212-1, Instructions to Offerors-Comc'l Items, applies and the following addenda: para (bZ)(4) In addressing technical portion of RFQ, offeror must provide their plan to accomplish work, schedule, and labor plan.
