SYNOPSIS/SOLICITATION for commercial items in accordance with the
format in Federal Acquisition Regulation (FAR) Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. Solicitation
Number FOH-99-J24 is issued as a Request for Quotation, under
simplified acquisition procedures, test program, unrestricted. All
responsible sources may submit a proposal. All timely proposals will be
considered by the Government. The SIC code is 7375 and the small
business size standard is $18.0 million. The Government intends to
award a single contract to the responsible offeror whose total offer
for all line items is evaluated and determined to be the best value for
the Government, price and other factors considered. The solicitation
document and incorporated provisions and clauses are those that are in
effect through Federal Acquisition Circular 97-13. The following is a
description of the requirements for this acquisition: BACKGROUND --
The Division of Federal Occupational Health (FOH) is continuing a new
product line called the Online Health Promotion piloted in Fiscal Year
1999, which is a tailored, interactive, Internet-based service which
offers FOH's customer Agencies an innovative, cost-effective approach
to reaching large geographically dispersed populations with a
personalized health risk appraisal, sound health information and health
improvement programs. DESCRIPTION -- The purpose of this requirement is
to provide a commercially available subscription-based, continuous
health promotion service to FOH's customers via the Internet. The
Contractor shall provide a website that shall empower the covered
employees to pursue better health. The Contractor-provided online
program shall include the following components: (1) Reliable health
information, tools for personal health management, and up-to-date
medical news -- all tailored to fit the user's individual interests and
needs; (2) Help employees identify health risks, change behaviors and
achieve personal health and wellness goals; (3) Allow users to use the
program to set and achieve goals including but not limited to smoking
cessation, weight management, stress reduction, pregnancy and fitness;
(4) Provide a comprehensive health risk appraisal, interactive health
behavior change modules with push messages including E-mail-based
messages combined with print reinforcement and incentives; and (5)
include a medically monitored "chat room" for users to share
information. Additionally, the Contractor shall: (1) Provide promotions
to motivate members, an online self-care flow chart for determining
health care actions and a drug database, as well as general up-to-date,
scientifically sound medical information; (2) Provide aggregate reports
on subgroups of FOH's customers to indicate the number of accounts
created by eligible members, number of repeat visits, page hits by
subgroup and cumulative reports of multiple subgroups. Plus aggregate
data from the health risk assessment.; (3) Provide a website from the
Contractor's own server that is maintained at a high level of security
to protect the medically confidential information that will be
collected from the users and that can be customized for FOH and its
customers; (4) Have a privacy policy showing their commitment to making
the personal health information private and secure; (5) Be prohibited
from releasing any personally identifying information to any person or
organization; (6) Have in place and use industry-standard security
technologies to avoid unauthorized access to collected information. FOR
EXAMPLE PURPOSES ONLY, use encryption-Secure Socket Layer (SSL) to
scramble the data as it's transmitted over the Internet to prevent
others from accessing the user's private account; if the user hasn't
usedthe program for more than 20 minutes, the program would
automatically log the user off; and (7) Be prohibited from selling or
advertising on the pages provided to FOH's customers. The online
program services (member access to the website) shall be available 24
hours per day, 7 days per week. Proposals will be evaluated based on
the factors as listed below for a base year and one option year, to be
awarded no later than 9/30/99, with an effective date for performing
services of October 1, 1999. Exercise of the award of a contract
resulting from the combined synopsis/solicitation is subject to the
availability of funds for each new fiscal year. The place of delivery
shall be FOH customer agency sites located throughout the United
States, Pacific Islands, Virgin Islands, and Puerto Rico. OFFER/PRICING
SCHEDULE -- CONTRACT LINE ITEMS: Contractors shall complete the unit
price and extended price for all line items including annual totals for
the Base and Option Years. A grand total for Base and Option Years
shall also be presented. Offerors should note that the number of
members stated herein will vary over the course of this contract. The
numbers providedfor each contract year is the anticipated maximum
number. BASE YEAR CLINS: Set Up Fee: $_____________; Member

anticipates approximately 50,000 members during the Base Year). Reports

solicitation: FAR 52.212-1 Instructions to Offerors-Commercial Items
and FAR 52.212-2 Evaluation-Commercial Items -- Evaluation factors
include (a) technical capability of the contractor to meet the
requirements of this solicitation, (b) past performance, and price.
Technical and past performance, when combined, shall be more important
than price. Proposals, submitted in response to this combined
synopsis/solicitation, shall include current product literature and a
copy of the Offeror's most recent commercial price list. Offerors are
required to submit past performance information, including points of
contact, on the last three contracts for similar services; a completed
copy of the provision FAR 52.212-3 Offeror Representations and
Certifications-Commercial items, must be submitted with the offer; FAR
52-212-4 Contract Terms and Conditions -- Commercial Items; and FAR
52.212-5 Contract Terms and Conditions Required to Implement Statues or
Executive Orders-Commercial items, with the following paragraph (b)
clauses indicated by number (1); (12); (13); (14); (15) and (23). The
closing date and time for offers submitted in response to this combined
synopsis/solicitation is September 10, 1999, at 2:00 p.m. local
prevailing time and shall be delivered by the specified time to the
