Description: This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation/proposals being
requested and a written solicitation will not be issued. This
solicitation F14614-99-Q5005, is being issued as an Request for
Quotation (RFQ). The solicitation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
97-11. This is a 100% small business set-aside. The Standard Industrial
Classification Code is 7389, with a small business being one with 500
or fewer employees. There are five (5)contract line items (CLIN). CLIN
1001 is Base Year, CLIN 1002 is First Option Year, CLIN 1003 Second
Option Year, CLIN 1004 Third Option Year, and CLIN 1005 Fourth Option
Year. The Government intends to evaluate offers and award a contract
without discussions. Therefore, qualified contractors should submit a
quotation with their best overall lowest price. The following factors
shall be used to evaluate offers: (1) price; (2) technical capability
of the items offered to meet the Government requirement. Offerors shall
include a completed copy of the provisions at FAR 52.212-3 Offeror
Representations and Certifications-Commercial Items with quote. The
Department of Labor Wage Determination 94-2215, Revision No. 11, dated
01 June 1999 will apply. The classification of employee status is
"WG-8 General Maintenance Labor" rated at $15.60 per hour. The Defense
Priority Rating to this procurement will be "DO-S10". The Contractor
agrees to comply with the following FAR clauses, which are incorporated
in this solicitation by reference, to implement provisions of law or
executive orders applicable to acquisitions of commercial items. IAW

to this requisition. SUMMARY OF WORK: The contractor shall provide all
tools, parts, labor, materials, equipment, transportation and
facilities necessary to perform the following requirements listed
below: a. Perform a weekly inspection of all equipment and identify
equipment in need of repair or maintenance. b. Respond within 24 hours
(excluding weekends) if called by Fitness Center Director (or his
authorized representative) to repair broken equipment. c. Re-upholster
all weight room equipment torn and in need of repair. d. Replace, at
contractor cost, all parts valued under $25.00. e. Replacement parts
exceeding $25.00 shall be submitted, in writing, by the contractor to
the Fitness Director prior to purchase for approval of the parts. f.
Replacement parts for equipment will be original equipment manufacturer
standard parts or a verifiable equal, and will include the billing
invoice with brief description, manufacturer part number/model number,
and cost. g. The contractor will coordinate the work efforts, and
purchases over $25.00 with Fitness Director. h. Only accept service
calls from the Fitness Center Director or appointed representative. i.
Contractor is responsible for safeguarding government property.
Contractor will properly sign out on an Air Force form 1297 any
property taken off base for repair. j. Perform routine maintenance on
all weight room equipment once a week at Bldg 412 (Fitness Center) and
Bldg 196 (Air Capitol Inn) McConnell AFB, per attached equipment below

4000PT Fitstep Stairmaster 1 0 FITSTEP WORK SCHEDULE: Work may be
performed during the hours of 5:00 AM and 11:00 PM excluding Saturdays,
Sundays, Federal Holidays, and base down days. The Contracting Officer
must approve work hours other than those listed. Note that
periodically Air Force crisis or emergencies may delay access to the
Base or the work area. In addition, contractor personnel may be
required to leave the work area and accomplish work at another time,
also vehicles are subject to search prior to entering and exiting work
areas or the Base. SAFETY: The contractor shall comply with current
OSHA, and Base safety/fire regulations. The contractor will be briefed
on all safety/fire regulations, emergency procedures, and requirements
relating to said subject before work commences. The contractor is
responsible for a safe and clean work environment during performance
and at the close of each business day. STORAGE: Government will provide
storage for parts, supplies, and equipment deemed necessary for the
maintenance for said contract. Storage will be coordinated with and
approved by Fitness Director or Facility Manager. SITE VISIT: OFFERORS
OR QUOTERS ARE URGED AND EXPECTED TO INSPECT THE SITE WHERE SERVICES
ARE TO BE PERFORMED AND TO SATISFY THEMSELVES REGARDING ALL GENERAL AND
LOCAL CONDITIONS THAT MAY AFFECT THE COST OF CONTRACT PERFORMANCE, TO
THE EXTENT THAT THE INFORMATION IS REASONABLY OBTAINABLE. IN NO EVENT
SHALL FAILURE TO INSPECT THE SITE CONSTITUTE GROUNDS FOR A CLAIM AFTER
CONTRACT AWARD. Site visit will take place on Friday, 10 September
1999, at McConnell AFB in building #732 small conference room at 2:00
