W -- PROVIDE AUDIOVISUAL SERVICES AND EQUIPMENT SOL 777-52-99 DUE 090299 POC Garry Plear (440)838-6028 The Cleveland Business Center, Cleveland, Ohio referred to hereinafter Employee Education Center (EES) proposes to enter into a contract for the rental audiovisual equipment and technical support for the annual Bioethics conference. This is a Request for Quotations, for which it and all incorporated provisions are those in effect through Federal Acquisition Circular 97-12. The procurement is 100% Set-Aside for Small Business. The Standard Industrial Classification Code (SIC) is 7359. The Small Business Size Standard for this procurement is $5 Million. This is a combined snyopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. The conference shall be held from September 8 through September 10, 1999. The location shall be the Radison Hotel South & Plaza Tower,Bloomington, MN. Complete setup shall be accomplished by 5:00 p.m. Central Time on Tuesday September 7, 1999. The Contractor shall provide the following personnel for 3 days each from 7:00 a.m.-5:00 p.m.: 1 ea. Camera Operator, 1 ea. Audio/Lighting Tech, 1 ea. A-V Manager/Coordinator to be responsible to the VA on-site Program Director. The Contractor shall also provide the following personnel for 3 days each for 2 ea. 2 hour sessions: 1 ea. Camera Operator, 1 ea. Audio/Lighting Tech. Striking shall take place at the conclusion of the conference. Setting for the General Session(Grand Ballroom West) approximately 250 participants: schoolroom/theatre; headtable, podium, on risers. Audio/Lighting control in rear of room on risers with mid-height pipe & drape surrond. Camera set on risers. The Contractor shall provide the following equipment to be utilized throughtout the conference: 1 ea. Sony DXC-357 or equal, 3 ea. CCD cameras(on risers) with studio configuration and long lens 30:1 for "talking head" shotfrom rear of ballroom, 1 ea. fluid head tripod, 1 ea. video lighting and dimmer board for image mag of podium area including backlight on drape (dimmer board setup near mixer in rear of ballroom) lights shall be hung from tracks or temporary grid bar or truss system, not trees, 1 ea. VHS Camcorder & Tripod for reference of slide and Powerpoint changes, 1 ea. VHS VCR(Playback), 1 ea. Beta SP Recorder with tape stock(14-SP 90 minute), 2 ea. 9x12 Rear Fast Fold Screens, 1 ea. Elmo TRV35-G Slide Projector (to be set up in rear of ballroom near camera), 1 ea. Wireless remote for above, 1 ea. Podium Mic, 2 ea. Panel Mics on head table, 4 ea. Audience Mics w/25' extensions coiled at base, 1 ea. Wireless Handheld Mic, 1 ea. 12 channel mixer(to be set up in rear of ballroom near camera), 2 ea. Video Projectors with remote -minimum 2100 ANSI Lumens(for XGA data & composite video), 1 ea. Elmo Visual Presenter, 1 ea. Panasonic 6 input vertical interval switcher(VCR, Elmo 35, Elmo Presenter, Camera), 1 ea. 8" Sony-2 input monitor, 1 ea. Flipchart, 1 ea. Pipe & Drape(Blue or Gray) across front width of ballroom(minus screen widths), 1 ea. Notebook Multimedia computer w/Win 98, Office 97, (full install) Pentium 233 Mhz, 64Mb RAM minnium, 3 ea. 2-way radios, There are 4 Breakout Rooms for use on three days (Veranda 1/2, 3/4, 5/6, 7/8). These sessions will be set classroom style with small headtable and tabletop lighted podium. Each Breakout Room will require the following equipment: 1 ea. LCD video projector( for true XGA data and Composite Video), 1 ea. 35mm slide projector w/bright light module, 1 ea. Wireless remote for the slide projector, 1 ea. Lav mic, 1 ea. 6x8 fast fold screen, 1 ea. Dress kit for the fast fold screen. Add the following equipment for 1 Breakout Room(Veranda 5/6) for 3 days: 1 ea. Sony DXC-357 or equal, 3 ea. CCD cameras(on risers) with studio configuration, with long lens 30:1 for "talking head" shot from rear of ballroom, 1 ea. Fluid head tripod, 1 ea. Video lighting and dimmer board for image mag of podium area including backlight on drape (dimmer baord setup near mixer in rear of ballroom) lights shall be hung from tracks or temporary grid, bar, or truss system-not trees, 1 ea. Beta SP recorder with tape stock (6-SP 90 min), 1 ea. VHS camcorder & tripod for reference of slide and Powerpoint changes. The Department of Veterans Affairs will retain rights to all-intellectual property recorded during the course of this conference. The following FAR provisions apply and are incorporated by reference: FAR 52.212-1 INSTRUCTIONSOF OFFERORS-COMMERCIAL ITEMS(AUG 1998), FAR 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS(MAY 1999), 52.212-4 CONTRACT TERMS AND CONDITIONS- COMMERCIAL ITEMS(MAY 1999), FAR 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS-COMMERCIAL ITEMS(MAY 1999) with the following numbered paragraphs checked: a (1) and (2), paragraph b (6) through (10), paragraph c (1) and (2), and paragraph e; VAAR 852.219-70 VETERAN-OWNED SMALL BUSINESS; and FAR 52.212-2 EVALUATION -COMMERCIAL ITEMS(JAN 1999) with the following evaluation criteria: 1) Contractors ability to meet requirements, 2) Technical experience, 3) Past Performance, a list of at least three agencies/companies (include addresses, telephone number and points of contact) for which you have provided similar services within the past three (3) years. 4) Price. The evaluation criteria are considered equal in importance with price. Please acknowledge all requirements on proposals. No partial quotes will be allowed. No substitutes will be allowed. A price proposal which includes all cost including travel, prompt payment terms remittance address, synopsis addressing evaluation criteria, a completed copy of FAR 52.212-3 and VAAR 852.219-70 shall be submitted responding to this request are due by the COB September 2, 1999(4:30pm EST).
