This is a combined CBD synopsis/solicitation for commercial items
prepared in accordance with the format in Subpart 12.6, as supplemented
with the additional information included in this notice. This
announcement constitutes the only solicitation; quotes are being
requested and a written solicitation will not be issued. This
solicitation, F14614 99 Q5012, is being issued as a Request for
Quotation (RFQ). The solicitation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
97-5. This is a 100% small business set-aside. The Standard Industrial
Classification is 8011, with a small business being one with 500 or
fewer employees. There are five Contract Line Items (CLINS). The
contractor shall provide all medical records coding personnel to
perform quantitative and qualitative analysis on a variety of
outpatient records by examining the records for completeness of
required date; performing generic screening on outpatient records;
assigning proper diagnostic and procedure codes including Current
Procedural Terminology (CPT4) codes and International Classification of
Diseases (ICD-9-CM) codes. Services to be performed at Bldg 250 (Super
Clinic), McConnell AFB, KS. CLIN 0001: For the basic period of 01 Oct
99 _ 30 Sep 00, approximately 15-20 hrs per week; CLIN 0002: For the
first option year the period 01 Oct 00 _ 30 Sep 01, approximately 15-20
hrs per week. CLIN 0002 For the second option year the period 01 Oct 01
_ 30 Sep 02, approximately 15-20 hrs per week. CLIN 0003. For the third
option year the period 01 Oct 02 _ 30 Sep 03, approximately 15-20 hrs
per week. CLIN 0004. For the fourth option year 01 Oct 03 _ 30 Sep 04.
The Statement of work is: A. The medical records coder must have a
minimum of an Associates Degree with an emphasis in medical records.
The contract medical records coder shall possess a minimum
certification as an Accredited Records Technician (ART). Certifications
such as Certified Coding Specialists (CCS) and Registered Record
Administrator (RRA) are not required but highly recommended. The
medical records coder shall have provided medical records coding
services within the past two years. B. The Contractor shall provide
medical records coding and consultation services approximately 15-20
hours each week based on the workload. These services will be provided
at the 22d Medical Group during hours mutually agreed upon by the
contractor and the 22d Medical Group. C. Accurately codes and inputs a
wide range of diseases, injuries, manifestations, ambulatory
surgeries, and non-surgical procedures onto the Ambulatory Data System
encounter forms and/or third party collection forms, in accordance
with Volume I, II and III of the International Classification of
Diseases, 9th Revision, Clinical Modification Codes (ICD-9CM). D.
Corrects record errors and forwards correct data to appropriate parties
involved. Ensures that diagnostic/operative terminology used is
consistent with current acceptable medical nomenclature. Explores
apparent inconsistencies or discrepancies with the responsible
provider. E. Properly sequences data, i.e. principal diagnosis,
secondary diagnoses, complication in order of importance, etc., to
optimize use of ICE-9-CM codes and CPT4 codes. F. Reviews entire
medical record, in addition to prepared Ambulatory Data System
encounter form, to ensure all information is coded and entered
accurately and completely. When omissions or inconsistencies are
identified, makes preliminary correction/additions and returns to
provider for concurrence. G. Audits outpatient records to ensure
internal consistency H. The contractor shall perform typing duties
associated with the coding and auditing of outpatient records.
Responsible for proper formats punctuation and spelling. I. The medical
records coder shall respect and maintain the basic rights of patients,
to include but not limited to personal dignity. J. The medical coder
shall attend and participate in meetings, professional staff
conferences, and other appropriate professional activities only as
necessary. K. The medical records coder shall serve in an advisory and
teaching capacity and shall conduct inservice training to providers
and clinic support staff on medical coding, as needed. L. The estimated
workload for the medical records coder: 1. Codes approximately 60
records per day where health care provider writes in the description of
diagnoses and procedures on the ADS encounter form. 2. Audits a minimum
of 5% of all medical records and ADS forms from each clinic;
approximately 20 records total per day. 3. Teaches approximately one
medical records coding class per quarter. M. Point of Contact for the
Medical Group regarding this statement of Work or day-to-day operations
will be the Team Chief, Medical Operations Administrative Element, or
designated representative. The provisions of FAR 52.212-1,
_Instructions to Offerors _ Commercial Items_, applies to this
acquisition. The Government intends to evaluate offers and award a
contract without discussions; therefore, qualified contractors should
submit a quotation with the best possible price. Provisions at FAR
52.212-2. _Evaluation _ Commercial Items,_ apply to this order. The
following factors shall be used to evaluate offers: price alone.
