This is a combined synopsis/solicitation for commerical items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement consitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request For Quotation (RFQ) F08620-00-Q0001. The solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 97-13. This requirement is issued on an unrestricted basis; the standard industrial classification code is 4812 and the small business standard is 1,500 employees. Description of requirement: Provide cellular telephone service for an estimated quantity of 270 government owned phones at Hurlburt Field Fl 32544, effective 1 Oct 99 through 30 Sep 00. FOB point is destination. Service required: 1) monthly access, 2)additional local area minute rate, 3) voice mail, 4) call waiting, 5) call forwarding, 6) caller ID, 7) three party conference calling, 8) mobile messaging, 9) conversion, 10) roaming, and 11) detailed billing statements. Service shall be available in digital or analog service. The following Federal Acquisition Regulations (FAR) provisions and clauses apply to this solicitation and are incorporated by reference: FAR 52.212-1, Instructions to Offerors_Commerical Items; FAR 52.212-2, Evaluation_Commerical Items; FAR 52.213, Offeror Representation and Certifications_Commercial Items; FAR 52.212-4, Contract Terms and Conditions -- Commerical Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders. FAR 52.212-3 is completed so that the following clauses under paragraph (b) apply: 6,7,8,9,13, and 17. Quotes must show at a minimum; (1) The name, address, and telephone number of the offeror; (2) terms of any express warranty; (3) items 1-11 seperately priced; (4) Representations and Certifications at FAR 52.212-3 and DFARS 252.212-7000; (5) past performance information to include recent and relevant contracts for the same or similar serivce (including contract number, points of contact with telephone numbers) and (6) quotes submitted are to include a statement specifying the extent of the agreement with all terms and conditions, and provisions included in the solicitation. The quote must be signed by an official authorized to bind your company. Offerors that fail to furnish required representation and certifications, or reject the terms and conditions of the solicitation, may be excluded from consideration. FAR 52.212-2, Evaluation_Commercial Items, is amended to read: _The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) past performance and (2) price. Price is less important to the other factor when combined. Provide Cage Code number.
