unrestricted; all responsible sources may submit an offer. This is a
combined synopsis/solicitation for commercial items prepared in
accordance with FAR 13 and the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice.
Solicitation number/RFQ N00244-99-T-B345 applies; an electronic copy of
the RFQ can be accessed at http://www.neco.navy.mil. This solicitation
document and incorporated provisions and clauses are those in effect
through all Federal Acquisition Circulars and Defense Federal
Acquisition Regulations Supplement 1998 Edition including updates
through today's date. The standard industrial codeis 7629 and the
business size standard is $5.0 million (annual sales). The agency need
is for a full-service, on-site maintenance contract for ten (10)
different pieces of Steris medical equipment, for a base year
commencing 10/1/99 through 9/30/00, plus three (3) option years. All
equipment is located at the Naval Hospital, Camp Pendleton, CA
(92055-5181). STATEMENT OF WORK: 1. Contractor shall provide all labor,
materials, parts (including software or software updates), components,
equipment, transportation, and supervision necessary to perform
inspections, and to complete preventive and corrective maintenance on
government-owned Steris sterilizer equipment. 2. Contractor agrees all
services will be performed on-site by trained and certified
technicians qualified to work on the systems. No piece of equipment
will be removed from Naval Hospital, Camp Pendleton without written
authorization from the head of biomedical repair or the plant account
clerk. 3. Contractor shall perform four (4) preventive maintenance
inspections during the base and option years. The inspections (base
year) are to be conducted in December 1999, March 2000, June 2000, and
September 2000. Times and dates shall be coordinated with Central
Processing Dept (CPD) and/or Main Operating Room (MOR) dept. a)
Preventive maintenance inspection covers such items as, but is not
limited to, lubrication, cleaning, mechanical and electrical
adjustments, functional tests, and verification of electrical
calibration, performed to the original equipment manufacturer (OEM)
specifications; b) Preventive maintenance inspections shall include the
replacement of all worn and/or failed components, including worn or
frayed electrical wires or power cords. Where statistical data on
similar equipment indicates failure point is approaching for any
component, said component shall be repaired or replaced in advance to
prevent system downtime or failure. 4. The contractor shall supply all
replacement parts and materials, except consumables, used during all
preventive and corrective maintenance at no additional cost the
government. Replacement parts must be new, certified, and in compliance
with manufacturer's specifications. Contractor shall use only
OEM-approved replacement parts and lubricants. 5. There shall be
unlimited emergency services during normal working hours, 08:00 a.m. To
05:00 p.m., Monday through Friday, holidays excluded. Response time
will be within 24 hours from receipt of the maintenance request.
Overtime work (after 1700, weekends, or holidays) will not be
authorized under resulting contract. If overtime is necessary, it will
be covered under a separate purchase order and will be at the
prevailing company rate. 6. Contractor will make any engineering
changes or updates to the equipment as required by the manufacturer or
regulatory agencies for operator/patient safety or increased equipment
reliability at no cost the government. 7. Prior to any service being
performed, the service representative will report to the Biomedical
repair section, Room G061,Bldg H-100, Naval Hospital, Camp Pendleton.
A copy of the completed service report will be furnished to biomedical
repair. The service report will include, at a minimum, the following
information: a) PM and or control number; b) Model and serial number;
c) Labor hours expended; d) List of parts used and cost; e) Brief
summary of repairs, inspections, or maintenance; f) Printed name and
signature of tech performing service; g) Printed name and signature of
designated dept rep. 8. Billing to be in arrears. 9. Persons
authorized to place calls against this order: a) Head, Biomedical
Repair or his/her designated representative; b) Head, CPD or his/her
designated representative; c) Head, MOR or his/her designated
representative; d) B. B. Sanders, Supervisor, Contracting Officer; e)
M. Brown, P.A.; f) J. Clark, P.A.; g) B. Lowe, P.A.; h) E. Gabut, P.A.;
i) C. Pugh, P.A. Note: Parties responding to this solicitation may
submit their standard/commercial service agreement terms and conditions
for review and approval.Such terms and conditions are subject to review
by the customer, and, if not in conflict with their requirements, may
be incorporated into any resulting contract. This requirement is for a
fixed priced contract for 44 line items as follows. Quoters should
price in accordance with their standard billing procedures; e.g. if you
bill monthly, pricing should reflect monthly charge for each piece of
equipment. Please indicate actual method, e.g. $000.00 per
month/quarter/annum. Also note: the government pays ONLY in arrears.
