Eschels, (734)439-3608 This is a combined synopsis/solicitation for
commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued.
(II)The solicitation is issued as a Request for Quotation, number
117-0571. (III)The solicitation document and incorporated provisions
and clauses are those in effect through FAC 97-12. (IV)This
solicitation is a 100% small business set aside. The SIC code is 8049,
the small business standard is $5.0 million. (V)Provide dental
hygienist services to the inmates housed at the Federal Correctional
Institution and the Federal Detention Center, Milan, Michigan. 600
sessions estimated per year. A session is a two hour time period. This
solicitation includes four option periods, requiring 600 sessions per
option year. (VI)The hygenist shall be licensed by the State of
Michigan and a graduate of a dental college curriculum. All sessions
will be performed during the normal operating hours of the institution,
Monday- Friday, 7:30 am- 4:00 pm. The hygenist will be escorted at all
times while inside the secure perimeter of the institution. The
hygenist will be in contact with convicted felons and shall conduct
him/herself in an appropriate manner at all times and shall report any
infractions of the rules & regulations by the inmates to the
institution staff immediately. The hygenist shall adhere to all
regulations prescribed by the institution for safety, custody and
conduct of inmates. The hygenist shall attend an institution
orientation program prior to assuming institution involvement.
Reimbursement is authorized at the contract rate, prorated hourly. The
hygenist shall clear a NCIC background investigation or other security
checks as required by the bureau of prisons prior to assuming
institution involvement. This appointment is a contractual arrangement
and not a personnel appointment. Payment is based on the provision of
an end product or the accomplishment of a specific result. The service
does not constitute an employer/employee relationship. The hygenist
will not be subject to government supervision, except for security
related matters. However, the hygenist performance shall be closely
