@ 612/370-2109 This modification deletes the Statement of Work in its
entirety and substitute the following: Statement of Work -- Labor
Contract 1. Description of Services a) Furnish four (4) laborers and
one (1) laborer/driver Monday through Friday (excluding holidays)
between the hours of 7:00 a.m. and 5:30 p.m. One of the laborers are to
be a working supervisor, with pager. At a minimum, a pager is required
for one laborer stationed in Washington, DC. All other laborers should
have a pager. The eontractor's personnel will perform in the
Washington, DC, area and Riverdale, Maryland, and may be occasionally
required to perform in the Beltsville, Maryland, area or within a
50-mile radius of Washington, DC. Two of the laborers will be housed in
the DC complex and three (including working supervisor) will be housed
in the Riverdale complex. Contractor to provide required hand tools,
hand trucks, and equipment. All carts, dollies, hand trucks, etc. must
have some type of protection on them to prevent them from damaging the
walls. Rubber is preferred. (1) Duties involve one or more of the
following: (a) Loads and unloads various items on or from a truck (b)
Receipts and verifies materials (merchandise) against receiving
documents, noting and reporting discrepancies and obvious damage (c)
Distributes materials to various delivery points (d)
Assembles/disassembles, delivers, moves and places furniture, supplies
and equipment (e) Makes minor furniture and carpet repairs and
cleaning (f) Removes installed carpet, padding, drapes, venetian
blinds, pictures and clocks (g) Operates hand/power trucks and tools
(h) Maintains a neat and orderly work area (i) Performs other related
duties as assigned (j) Makes door signs (k) Provides laborers who are
able to read, write, and understand English (1) Provides certified time
and attendance sheets for the laborers at the end of each pay period,
and establish and maintain an employee sign in/sign out log (m) Return
completed signed/dated work orders to COR at end of each workday (n)
Makes adjustments to existing systems furniture Furnish one 1/2ton
panel van on Mondays and Wednesdays between the hours of 7:00 a.m. and
5:30 p.m. If van day falls on a holiday, COR will determine if van in
needed on another day instead. (1) Duties for van driver (a) Drives
van to transport materials, equipment, furniture, and/or workers
between various establishments such as Government facilities,
warehouses, places of business, etc. (b) Loads and unloads van with or
without helpers (c) Functions as a laborer when not driving van (2)
Van and driver to be used solely in the performance of duties relating
to the receiving room. (3) The Government will provide parking
facilities for the van within the USDA Washington, DC, complex. Parking
in Riverdale complex will be the responsibility of contractor. c)
Furnish truck and driver with padding and any additional equipment
(such as hand trucks, dollies, etc.) and additional laborers as
requested. No representation is made as to the frequency of this
service or the aggregate value of this service. (1) Order for the
service in c) above should be placed via telephone by the Contracting
Officer's Representative (COR) at least 24 hours prior to time of
required service. Each order shall specify amount of labor and
equipment required, location, time and name of Government
Representative to whom the contractor is to report. (2) Duties of truck
driver involve one or more of the following: (a) Drives truck to
transport materials, equipment, furniture and/or workers between
various establishments such as Government facilities, warehouses,
places of business, etc. (b) Assists laborers in the accomplishment of
assignment duties (c) Loads and unloads truck with or without helpers
(d) Works under the direction of the working supervisor (3) Duties for
laborers are as assigned by the Government's labor services supervisor
who is the Contracting Officer's Representative (COR). 2. Contracting
Officer's Representative The Government employee designated as
Contracting Officer's Representative (COR) or working supervisor shall
work with contract personnel to identify tasks to be performed and
shall certify on the "Labor Services Worksheet" as to the satisfactory
completion of the work requested and as to the total number of hours
of each category of labor and equipment utilized. One copy of the form
can be furnished to the contractor for submission with his invoice.
Any disagreement concerning the acceptability of the performance shall
be treated as a dispute concerning a question of fact and shall be
subject to the "Disputes" clause. BRAND OF REPLACEMENT TO GOVERNMENT
ITEMS DAMAGED BY LABORERS SHOULD BE AT THE DISCRETION OF GOVERNMENT
REPRESENTATIVE and shall be approved by the COR prior to replacement.
