synopsis/solicitation for commercial items prepared in accordance with
the format in FAR subpart 12.6 as supplemented with additional
information provided in this notice. This is a sole source acquisition
to Acuson Corporation of Mountain View, CA. Work is for inspecting,
servicing and preventative maintenance to Acuson Ultrasounds Systems.
Solicitation number F65501-99-Q-0199 is issued as a Request for
Quotation (RFQ). This announcement constitutes the only solicitation;
a quote is being requested and a written solicitation will not be
issued. The incorporated document provisions and clauses are those in
effect through Federal Acquisition Circular 97-12. The Standard
Industrial Classification Code is 7629, small business standard is
$5,000,000. DESCRIPTION: Contractor shall provide all labor, materials,
equipment, transportation, tools for inspecting, servicing and
preventive maintenance necessary to keep Ultrasound Systems functioning
in a safe, reliable condition. ITEM AND ACCESSORIES INCLUDE: AEGIS

ACUSON, SEQUOIA, Serial # 51453 LOCATION: 3rd Medical Center, 5595
Zeamer Avenue Elmendorf AFB, AK 99506 Rooms 1E109, 1E110, 1E114, and
1E121 PERFORMANCE PERIOD: 1 Oct 1999 -- 30 Sep 2000 with four option
years. REPAIR SERVICE HOURS: During normal duty hours from 7:30 AM
through 4:30 PM, Monday through Friday, except holidays observed at the
Government installation. In the event of an equipment malfunction, the
contractor will be notified by the Contracting Officer or designated
representative. Contractor shall respond via telephone to the
Contracting officer or designated representative within 2 hours after
receipt of telephone notification. The contractor shall service the
equipment within 24 working hours from notification time. PERMITS: The
contractor shall secure and pay for all required permits, fees and
licenses. WORK LOCATION: The contractor shall report to the Medical
Equipment Repair Center (MERC), Bldg 5955 Zeamer Ave., prior to
commencing work. Before beginning and upon completion of any scheduled
or unscheduled service call visit, the contractor_s service
representative will sign in and sign out with any representative from
the MERC. REPORTS AND DOCUMENTATION: The contractor shall obtain, file,
and make available all operational and technical documentation which is
necessary to keep the equipment at operational status. Upon work
completion, the contractor shall submit an inspection report to the
MERC. The report shall include: Contract Number, Name of service
technician, Repairs completed, Signature and date from authorized
Government representative QUALITY CONTROL: The contractor shall
establish and maintain a complete Quality Control Plan to ensure the
contract requirements are met. The plan shall be submitted for approval
to the Contracting Officer no later than the Pre-Performance
Conference. If the Quality Control Plan is changed, an updated copy
must be resubmitted for C.O approval. TESTING AND INSPECTIONS: The
contractor shall perform all inspections, test parts and systems
specified herein, as stated in applicable codes and standards.
Contractor shall provide all necessary equipment, material and labor
necessary to properly perform the above. All testing and inspection
shall be observed by the Contracting Officer or the authorized
representative. The contractor shall adhere to all ultrasound imaging
and Doppler safety practices as established by the permiter (3rd
Medical Center, Elmendorf AFB, AK). EQUIPMENT: Only equipment in good
serviceable condition is to be covered by this contract. Equipment
shall not be rejected on the basis that it needs cleaning, minor
repairs, or adjustments. The contractor shall keep the system in good
operating condition. HARDWARE AND SOFTWARE PROGRAM: The contractor
shall perform maintenance utilizing hardware and software corrective
maintenance which includes: complete set of special service tools,
test, equipment, fixtures, verification procedures and schedules to
provide operational and diagnostic verification of system performance,
software problem analysis, associated reprogramming and updating
system software. FREQUENCY OF SERVICE/RESPONSE TIME: a. Scheduled calls
will consist of 2 (two) semi-annual preventive maintenance/calibration
inspections to be performed in the months of January and July. All
scheduled maintenance visits will be coordinated with the Ultrasound
Department at least three working days in advance. Phone (907)
580-6796. b. Unscheduled calls (during normal working hours) are calls
deemed necessary to maintain equipment in good operating condition.
For unscheduled maintenance the contractor_s representative is to
arrive not later than noon of the second working day after a service
request is originated. REQUIRED SERVICES: a. Scheduled maintenance
inspections shall include, but not be limited to the following: (1)
Perform operational check of all systems and correct any deficiencies
noted. Inspection should include complete diagnostic testing of
transmitter, image formation, control section, and verification of
pulsed/color Doppler_s. (2) Verify that the system_s internal and
external calibrations are within manufacturer_s specifications.
Verification should include reviewing operating system software
diagnostics. (3) Clean and lubricate mechanical parts in accordance
withapplicable manufacturer_s service manual. (4) Perform an electrical
check out of the instrument and all accessories included in the
contract and make electrical adjustments, as required, to insure
compliance with manufacturer_s specifications. (5) Replace excessively
worn or defective parts: Excessively worn parts are those parts that
if not replaced immediately may fail prior to the next scheduled visit.
(6) Visually inspect exterior of all systems for damage and cleanliness
and report any discrepancies to MERC. (7) Make mechanical adjustments
to insure proper operation. (8) Consult with the equipment operator
regarding equipment performance. b. Unscheduled maintenance: The
contractor shall provide on-site repair support to correct all
malfunctions which may occur. All tools, test equipment, parts and
supplies necessary to return the equipment to original operating
specifications shall be the responsibility of the contractor. Test
equipment will have a current calibration traceable to the National
Bureau ofStandards. REPLACEMENT PARTS: The contractor shall use only
new original or factory refurbished replacement parts in the
performance of this contract. If required replacement parts are not
immediately available to the contractor while performing scheduled or
unscheduled service calls, the contractor shall notify the MERC of the
deficiency and deliver to the hospital all replacement parts required
to complete the inspection in order to maintain a 95% uptime. The
contractor shall obtain written approval from the MERC before removing
item from the 3rd Medical Center for service at the contractor_s
establishment and provide a loaner like item at no additional cost to
the government. All maintenance labor and replacement parts shall be
accompanied with the contractor_s standard written warranty. In
addition, the contractor shall provide replacement transducers as
needed to meet FDA and UL standards. SAFETY: The contractor shall
comply with all Federal, State, and Base fire and safety regulations
and standards. A copy of these regulations can be obtained from each
department. The following provisions and clauses are incorporated and
shall remain in full force in any resultant purchase order; FAR Part
52.212-1; Instructions to Offerors, Commercial Items is incorporated by
reference. The provision at 52.212-2; Evaluation-Commercial Items,
applies to the solicitation. Award will be made based on past
performance and price being evaluated as substantially equal. The
government reserves the right to make award on the initial quote
received without discussions. Each offeror shall include a completed
copy of the provision at FAR 52.212-3; Offeror Representations and
Certifications -- Commercial Items with its quote. FAR Part 52.212-4;
Contract Terms and Conditions-Commercial Items, is incorporated by
reference and applies to this acquisition. Note the following addenda
to FAR 52.212-4: Incorporate the clause by reference at 52.212-5;
Contract Terms and Conditions Required to Implement Statutes or
Executive Orders -- Commercial Items. However, for paragraph (b) only
the following clauses apply: 52.203-6; Restrictions on Subcontractor
Sales to the Government, with Alternate I, 52.219-8; Utilization of
Small Business Concerns, 52.217-8; Option to Extend Services, 52.217-9;
Option to Extend the Term of the Contract, Paragraph (a) insert 15
days, Paragraph (c) insert 5 years, 52.222-26; Equal Opportunity,
52.222-35; Affirmative Action for Disabled Veterans and Veterans of the
Vietnam Era, 52.222-36; Affirmative Action for Handicapped Workers with

Privately Owned U.S. -- Flag Commercial Vessels. Incorporate DFARS
252.204-7004 Required Central Contractor Registration. Incorporate
DFARS 252.212-7001; Contract Terms and Conditions Required to Implement
Statutes or Executive Orders Applicable to Defense Acquisitions of
Commercial Items. However, for paragraph (b) only the following clauses
apply: 252.225-7001; Buy American Act and Balance of Payment Program,
252.225-7012; Preference for Certain Domestic Commodities, and
252.243-7002; Certification of Requests for Equitable Adjustment. The
Contractor shall extend to the Government their standard warranty
offered in similar types of sales. Acceptance of the warranty does not
waive the Government_s right with regard to the other terms and
conditions of the contract. In the event of a conflict, the terms and
conditions of the contract shall take precedence over the warranty. The
warranty period shall begin upon final acceptance of the items listed
in the schedule. A firm-fixed price purchase order will be issued under
the Simplified Acquisition Procedures. To obtain a copy of the bid
